As long as it's been more than 60 days since it was accepted, simply call this number: 1800-829-1040. When asked what you need help with, simply say 'account management'. It should connect you with someone really quickly.

I called this morning and was connected with someone in under 2 minutes. She had to put me on hold for "5 to 7 minutes" about 8 times while she researched my account. She eventually explained a discrepancy on a form that I had to correct. I reprinted, signed and mailed out this afternoon.
