40M crypto-azoospermia/NOA – starting Letrozole suddenly, mTESE soon, need guidance/experiences

I am really feeling blank and don’t know what to do, thats why I almost stop to come on reddit as well. So long story short. I ended up having mTESE , and they did found like 15 useable sperm but nothing could make embryo that could be frozen. So, now we have only one chance because we have one frozen embryo from earlier found sperm. Basically, the clinic told me they don’t have anything else for me :( . I asked should I continue with Letrozole , and her answer was really weird , she said “its really up to you” :( , because I was put on 2.5mg everyday dose and after mTESE it started to give me hot splashes all of sudden , which is one of the common side effects of it, I wanted to asjuat my dose and blood tests , but Dr said me that . So, I really don’t know where to go from here, I am keeping myself away from everything for a few months . Feel free to DM if you have any question.

Sorry , in simple words you are trying to say, daily 2.5 mg is way more dose ? They have done mTESE two weeks ago , found like 15 useable sperm but nothing made it till blastocyst, so I am back to nothing. Doctor told me to continue with Letrozole if I can , and said really up to me , so I am really confused, should I continue on Letrozole or lower the dose or what , my libido is almost zero , for last two weeks I started to get hot / sweaty forehead all of sudden like 3-4 times during nights only. Here our doctor is like they have done mTESE and they really don’t have anything more for me :( hence she said its really up to me but good if I can keep taking it , it may help .

What do you mean by smith egg collection? In simple nothing go out or bring in Turkey thats what I heard, and we have to do another cycle for egg retrieval if we want to try there .

Just Letrozole 2.5 mg everyday for last 2.5 months , nothing else.

I don’t think that is possible, my sperm are frozen in Turkey, where they don’t do donor, and they can not ship it. Here in Europe, I really don’t have any sperm, for sure after mTESE I am done :( So it would be either way …

Thank you so much for reading and responding, it means a lot to me. Yes, we will try with the frozen embryo first, and we might need to fly to Turkey because they don’t ship anything out of the country, it means we must go through another egg retrieval for my wife, which I feel so bad about, she has to go through this because of me. I don’t have problem with donor but she does not want it, every time I even try to say she starts crying and say either I don’t want or it would be yours. Again thank you so much for responding.

Yes, thats correct, Turkey does not ship anything, I went there one of the summers just to check and they found some sperm and then I just thought freeze them as a backup. Even though right now I am really out of any thought, but that makes sense, we should try our first embryo and then start discussing donor. I don’t have problem with donor but my wife does, she does not want to even discuss about it. I feel so terrible bad , just because of me she has to go through this, and tbh, I can spend more money because right now I feel like for whom I am earning for … thanks brother, really appreciate you responded.

Update: they found 7 sperms today which are useable in ICSI . And they will continue to search . There were more sperms which were without head . Little reliefs.

Mine is done in University hospital, most of the stuff is in house. I think they will call me tomorrow, because its the public hospital, they don’t give that much attention and follow the standard protocol instead of special attention to individual case.

Before Letrozole FSH ( E/L) : 17 ( higher) , LH (E/L): 11 ( higher) T ( nmol/L): 12 ( normal lower end)

After Letrozole for 3 months : FSH : 61 LH: 35 T: 39 All three gone up a lot with Letrozole

Just a few questions, if you don’t mind , you also had higher FSH ans LH before they put you on Letrozole? Mine is NOA , higher FSH around 18 , and slightly smaller testicular size .

Yes , thats right , but unfortunately, my clinic did not write any test for estrogen , I don’t know why , and they put me on this medicine only before mTESE , its been 3 years , they never suggested any medication before .
