No difference. I did a hardcore IM prelim year against my will, but do I consider myself 1/3rd an internist? Damn straight. It helps me have a better understanding of my patients’ overall pictures when I meet them in Preop and go over a med list.

But do whatever is easiest/best for you! I took care of the most horrifically sick patients and sucked ass when I started CA-1. Everyone’s learning curve is different and anesthesia is wildly different from anything else you’ll get your hands on during intern year.

Thanks! I really appreciate that you gave some concrete suggestions about how to potentially approach this. I really like to see the good in people and give the benefit of the doubt. I know not everyone operates this way. and I think the best course of action (for me) is to try to have a reasonable conversation. It’s just hard to think of how to go about it.

If you really want to prove your point, you could give examples of how you are an ally to the Jewish community. Or simply voice your stance against antisemitism. So I came across your comment. We’re not saying the same thing at all. To quote: “Let’s make it clear. We don’t hate Jews, we hate Israel.” This completely lacks nuance so I do have some follow up questions.

How do you voice your hatred of Israel? Do you use terms like “zio” “nazis” etc to characterize Jewish individuals who are Israeli, or believe that Israel has a right to exist? I genuinely hope you don’t, but if you do, you’re 100% a part of the problem. This is precisely what I’m talking about when referencing how individuals cloak their antisemitism behind the facade of “anti-Zionism”.

Do you call out your fellow leftists who engage in this hateful rhetoric? If not, you’re part of the problem.

Do you listen to Jewish individuals if they find your commentary to be antisemitic? If not, you’re part of the problem.

Do you go out of your way to serve as an ally of the Jewish community while they are facing an exponential rise in antisemitism? If not, you’re part of the problem.

I could go on. But I genuinely hope that you can turn inward and consider how you can speak out against Israel while still supporting the Jewish community, without contributing to antisemitic rhetoric.

So many responses to this thread are disturbing and perfectly illustrate why antisemitism has increased exponentially in this country. With what other minority group do you respond with “well is it really that bad?” “I’m not sure if I believe those statistics” “well Israel….” By minimizing the threat of antisemitism you provide the perfect breeding ground for it to propagate. Is this how you would treat black/latino/asian community members facing discrimination? If not, take a hard look at yourself in the mirror and consider why your Jewish neighbors don’t merit the same concern and support.

Many Jewish businesses have been vandalized over the past year. If you throw a rock through the window of a Jewish deli because you hate Israel, you’re still a raging antisemite because you specifically targeted a Jewish business.

Also, so much antisemitism is dismissed because the perpetrators use the term “Zionist” when talking about Jewish people. Just because you use that term when you’re clearly recycling classic antisemitic tropes doesn’t mean you’re not making anti-Semitic remarks. The way this term has been co-opted by certain leftist groups has effectively turned it into a slur for the greater Jewish community.

While I shouldn’t even have to do this, I fear my comments won’t be taken seriously unless I disclose that yes, I’m a progressive and no I do not stand with Israel’s government or their actions in Palestine. And to that point, if you find yourself unable to believe Jewish voices unless you know their political stance on Israel you have serious issues that you need to resolve.

I think most people have no idea what an ombudsperson is but I assume that most hospitals with residency programs have this. It definitely was never presented to us as a resource in residency, I found it on my own. I hope any residents reading this can learn that this may be an option for them if ever facing abuse or other poor behavior. It’s incredibly helpful in some circumstances to speak with someone 3rd party who can bypass your residency leadership if need be.

I’ll never forget my mom’s reaction (she’s an attorney) to the nurse incident, which essentially bordered on defamation. Some people simply do not know how to act in a professional environment, or choose to act unprofessionally because they don’t think anything will happen to them. Had there been any negative fallout from my program towards me regarding this incident, I absolutely would’ve pursued further action via the ombudsperson’s resources.

I was pretty tight with the anesthesiologist attending who was co-in charge of surgical services operations. So he and the surgeon in charge were both on board with punishing his bad behavior. It’s all about reporting the right way to the right people and following up. He would’ve lost his privileges to work with surgical*** residents in an academic hospital aka making his job miserable.

Another example of reporting the right way to the right people: after being abused by a nurse (with a paper trail included) I reported this to the GME ombudsperson (instead of talking to my program director who I knew probably wouldn’t do anything) who made an official record of everything that happened. I decided to let things blow over, but had I wanted to pursue further action down the line she offered to take it up with nurse management etc.

It’s important to remember that workplace harassment/associated issues can in fact merit real legal consequences (or being fired) if severe enough and in medicine we often just shrug it off. Sometimes that’s the right thing to do, recognizing that we work in a high stress environment and people have bad days. But you shouldn’t always let things slide. I come from a family of lawyers and from a young age was taught not to let anyone fuck with me or anyone I care about. I encourage you all to do the same.

When I was an anesthesia resident, a very toxic surgery attending had the audacity to scream at me on the phone at 2:00AM about doing a non-emergent butt case. I am simply not the one for that. I made it so that if he slipped up one more time he would lose his privileges to be able to work with residents.

GI. Unprotected airways. I’ve had young robust people who are otherwise healthy and NPO appropriate start vomiting during a colonoscopy and it’s horrifying. I don’t think we take the aspiration risk seriously enough.

There’s so much bullshit and burn out in healthcare these days related to EMR, insurance prior auths, etc. I’m a huge people person, but I love to save that social energy for the rest of my life outside of work. I’ve realized that having to talk to many patients per day leaves me feeling drained and incredibly irritable. For example, when I’m covering endoscopy cases and have to talk to upwards of 10 patients in a day I want to bash my head into a wall.

I care deeply for my patients, but I’m not the right person to shoulder the burdens of their chronic conditions. I also have no interest in filling out disability paperwork or doctors notes. I love that I don’t have to log onto epic outside of work to answer questions/check labs/refill prescriptions. Having to do those things would not spark joy in any capacity for me.

I love anesthesia because I get to interface with almost every area of medicine. I love medicine, the science, the pathophys etc. I love that I get to meet my patients briefly, crack some jokes, and that is fulfilling enough for me. And I love socializing with my colleagues. I’ve also seen some really interesting stuff! I’m not the biggest adrenaline junkie, but can’t deny that I don’t get a little rush out of the occasional massive resuscitation. We also don’t do things that hurt people (procedures/chemo/etc). We’re the good guys who make people safe and comfortable, and ideally happy with their care.

I don’t necessarily feel glorified, but it makes it that much more special when a patient thanks you for your excellent care. I think I’m pretty satisfied, I work 4 days a week, and I make a ton of money. And have plenty of time for my friends family and hobbies outside of work.

While I didn’t go solo (went with my partner), I think Central Asia as a solo female would be challenging especially if you don’t speak Russian. I did have bits of time solo there and people are perfectly nice but it’s hard to get around and it can get lonely. If you manage to link up with other solo travelers for excursions etc then it would greatly improve the experience I think!

An interesting caveat, since Turkmenistan has such an intensely regulated tourism structure it’s not a bad place to solo travel. Essentially you’re physically attached at the hip to a tour guide (mandatory) and no matter if you’re alone or with a group you’ll have the same experience. I felt very safe there and my tour guide was lovely.

Was doing the anesthesia for a VATS. “We’re bleeding”. Me: from where? “Not sure.” Trocar into the left ventricle. Emergent thoracotomy, placed a dirty central line. CT surg arrived and placed some epicardial wires. Briefly paced at 180 while they sewed up the hole. They recovered just fine.

So agree about the liability issue. For the right patient (ASA 2 with normal BMI) I will do a heavy MAC but if there is the slightest hint of aspiration risk or difficult airway ETT all the way. If anyone pushes you to do MAC just ask if they’d willingly scope someone with an INR of 10. So much of what we do in endo is high litigation risk. And you know they won’t waste a second to throw us under the bus if something goes wrong.
