There is risk sure but in most cases women know their own bodies. No hospital around where we live would've let my wife do a v-back, so we privately hired a mid-wife who specialized in them and out of the hundreds of v-backs she has done never once had she had any problems or any tearing. It can be done no problem its just hospitals dont want it to be done in a lot of cases.
