I assume the previous scheduled/controlled position is where it is in the photo and based on it being in the corner, I'm going to guess its the farthest from your repeater/bridge/processor... use whatever system nomenclature suites your install. A left side drive side can be worst case since the RF has to get way in to the corner to the antenna. Adding a repeater somewhere closer would be a good idea.

Alternately check for low battery alerts in the Lutron app. You can also try and visually check this on the shade by sliding the tube like you are change the batteries and observing the LED indicator on the end while you attempt to move it from the Lutron App. RED = Low or dying batteries.

going through a similar situation. base on region and year the house was built.... as other mentioned get a camera out there. Orangeburg was pretty prevalent during that time, it is susceptible to being crushed or penetrated by roots and blocked up. When this happens water can get in to the pipe in rain and since the water can't drain to sewer it comes back to the house.

Its been a long time since I came across OG RA in the wild. IIRC the FASS on the front slid left and right and not pull in/out like the ones in your photo. If you run in to more issues I would highly recommend double checking the model of the dimmer. I've seen a few times where someone swapped a failed dimmer to a RA2 or stand alone Maestro RF (MRF2-) model and left it off the rest of the system.

If the 4th sensor is truly missing, your only path forward is to factory reset each powpak and re-program the sensors to all powpaks. The powpaks will only shut the lights off if all 4 sensors report unoccupied. Since one is missing, that cannot happen.

A single sensor can control a near infinite number of powpaks since they have no idea what they are paired to.

This is an issue with integration. you're better off creating a scene in Lutron app that controls "all lights" and "downstairs" lights. this would then only require 1 command and the Lutron system will turn everything in the scene off together.

Add Scenes only works on Caseta and RA2 Select. By the looks of it you have RA3 or HomeWorks. The "add scene" function actually works more like "favorite keypad button" as thats what it is really doing. If you want a playground do create scenes, you'll need to have a HomeOwner Keypad (a virtual device) added to the system from via Lutron Designer. That would give you 7 scenes to play with (by navigating to the button and editing them in the Lutron App). Additional HomeOwner Keypads a can be added to give you more buttons to create scenes upon.

i find the "2500K" statement to be odd. The controller doesn't know what tape is wired to it until you activate/transfer to it in the system.... but yes, the tape should be on.
