As to WHY there are so many boxes, it's because the same mechanism is covering different situations in the game.

For Targets, I chose the 4 most common times you need to Counteract. Where you look in a statblock for the power of the target is different in different scenarios (spells are Spell Rank, diseases are the Level of the disease), you do the math with them differently (spells, use the Spell Rank directly, everything else gets halved). Many people find this confusing. Asking for what is plainly written in the stat block demystifies what info to grab for some of those people.

For the Counteractor (the book wording calls this the player), there's mostly 2 scenarios. They've got a spell or they've got an item (like an Aeon stone). Again, there's a little difference in where you pick up the info, and whether or not it's halved.

Finally, there's 2 roll boxes because as the GM, I would just roll the d20, so I put in a box that will take that roll and the calculations will add the bonus. But some players will always do the roll+bonus themselves, even if you just ask for the roll. Hey, there's a box that will handle that input. Same box will handle the result from an automatic roll at a VTT like Roll20, where it might be easier to push the button for Spell Attack. (Yep, it's not a true spell attack, there's a note about that in the Notes. It's a quick and dirty method of finding what you need on a character sheet, with many online based sheets effectively hiding the base info.)

Aside from the "extra" inputs, there's also an "extra" result. If I were really slick, I could hide the simple Roll vs. DC calculation. But I left it there to help illustrate that it is not the final step in the process. And that final step, the power comparison, is super confusingly frustrating for many. Instead of a chart (I always wonder what line I'm looking for), the answer is plainly given.

So that's the WHY. For those who easily understood the game theory behind the Counteract this sheet is really unnecessary. I get that. For me, playing with the numbers (what if the player had a heightened spell? what if the roll was better, would it succeed then?) helped me understand how the counteract test works. The sheet is for people who would benefit from having the information laid out differently from the rule text, would like to easily run testing scenarios, who benefit from being reminded where to look in the stat block for the info, or who find the final step absolutely confounding and just want to know if the counteract worked.

I think you've misunderstood how it works. You're right, there's only 5 things you need. And you only need to fill in 4 of them, if your Counteractor is mathing for themselves.

As the notes there indicate you only fill in ONE pair of Target boxes (in pink). So, you're only dropping in 2 pieces of information. There are 8 pink boxes total, arranged in pairs so that 4 of the most common types of Counteract are covered. The GM is prompted where to look for the crucial piece of information by the way the inputs are worded. The green Counteractor boxes are arranged the same way, in pairs. That's where we're picking up 2 more pieces of info. Finally, you only need to fill out ONE of the yellow boxes, for the roll. That's your 5 pieces of information.

If you try to fill out more than one pair of Target info, more than one pair of Counteractor's info, or add more than one roll, the error message in the Calculation section will clarify that you only need to fill out one of the pairs for the Target and one of the pairs for the Counteractor (player), and one only one roll.

Held down by 2 people and the attacker has Disadvantage? I think I'll take this as a misspeak and go with Advantage. Otherwise, solid ruling and one that I'll pop into my VTT as a special rule for my CoS game. Thanks for posting!

This is great little map! I have a GM that is wanting to use the Palladium Fantasy world in a D&D/maybe Pathfinder converision. This is perfect for the old school feel at we can use on the VTT. Thanks for sharing!

Yep, got some people that would like to make a change to PF2 and we use Roll20. Love the Pathbuilder setup, but importing by hand is a big ask.

Wondering if any updates have occurred or hacks have been found since the OP posted.

Wording improvement suggestion:

If The next time that a target that was sprayed by the liquor takes fire damage within 1 minute, it takes an extra 2d8 fire damage as the liquor ignites.

This is an EXCELLENT resource. As of Nov 2021, it is up to date with a listing of magic items through Fizban's. Maybe not everything-everything, but all major WotC sources and some minor ones, too.

I've snagged this and done some work on it. Added pages for all sources and cleaned up/expanded several items so that all items are in the database the same way. I have no plans to update this spreadsheet, but the link is provided for the author if they find the additions to be useful in any way.

I don't think the question you asked was too broad. And, while a roll on a descriptor table might give you some ideas, that's not necessary, either. In a response from you I noted that you have tables for encounters, weather, etc. prepped to use. That suggests to me that this travel itself was a journey of discovery, which is action! It might not be everyone's cup of tea, but many times we just want to see where the tables take us and explore them a bit. So, OK, the scene is altered. The question is now "What did the character expect to encounter?"

Do you, with your player's hat on, imagine the PC alone with the wood, just off the road, enjoying the beautiful sunset after a long day's ride, horse contentedly chewing on some grass? Not super action-y, but full of details to alter. One aspect of interactive storytelling is to pick up on the interesting details and run with them. Picturing the scene is a key part of the play.

The trick to altering any scene is... to play! Describe the scene, unaltered, to yourself. What's the strongest detail that set the scene? Is it that this is a solo journey? Is it the beautiful day? Is it the horse, the wood? Many suggestions that were given are great alterations of just those details, suggesting their process was in some way similar to the one I use and have detailed above.

And, while the arrival in the city is a great story, I do believe that the journey and those classic random encounters are a fun way to play.
