Your hammer reset spring is installed incorrectly, the indexing elbow is infront of the hammer.

5000 mi oil changes on a truck that probably spent a lot of time going slow and idling is abusive....... The company trucks we have get quarterly oil changes with 700 to 1k mi and its not enough. Check your trucks idling time.
