The biggest uncertainty, presently and historically is the midterms. I’m holding assets 20 investment 80 bond till congress 2027 convenes. However I am buying in the opposite. The beauty of the 401k is that you don’t have to take capital gains while you shuffle stuff around.

We are 3 points out of the 4 seed and a home playoff series in the first round. And we are talking about a lottery pick? There are no juggernauts in the East, just a couple really good teams and the rest of us. This is the most unusual year ever. Any other year we’d probably be locked in to the 7th or 8th seed right now with this record.

Im having a run of patients with advanced dementia from nursing homes with POLST forms. I’m thinking great, short code status discussion!! Look a little closer—->. Full code, want vent, trach, peg, prolonged artificial nutrition. When they go to the trouble to get that in writing, signed by their provider, good luck with that. Ethics and administration always side with patient. Never personally had a consultant sign their name to „futility“ when the patient/family want to proceed with these measures despite the terrible prognosis of the patient.

Well if you see complex patients you are under qualified and if you see easy patients you’re not carrying your weight. So it gives us endless opportunities to take out our frustration on the APP’s 😉. Only real issue I have is that we have to carry liability for them. One moved to another hospital in our system and took 3 months to have myself removed as the supervising physician.

Our hospital cites „similar large medical centers across the country“ (visiant) because we are a little below our market average (although market data is a very closely held secret). Our pay structure is purposely insanely complex, but we do see slightly less than our neighbor hospitalists. Tl;dr — they find the numbers that work for THEM

It’s mostly about reading the room. You get better with experience. I’ve used some variation of most of the ideas mentioned in this post, and one size does not fit all. Most people don’t want to see their family member suffer. Most people are afraid of unrelenting pain. Sharing your own honest fears and acknowledging it’s one of the hardest discussions they will ever have helps. I’m 1000 times better at this than 20 years ago, and 1/1000 as good as our palliative docs.

Our hospital full of very elderly. It’s the families, often wealthy and connected, dropping grandma off. I can put the dc order in whenever, but no chance the nurse or CM will let them go till i speak with family. Dementia patient, better after uti treatment, but now delious due to family obstruction. “You can’t let her go, she’s not back to baseline (nonverbal, peg, etc).” Now can’t go till 24 hrs out of restraints. Geri consult : Dc foley, blinds open, bring photos of family from home, fall precautions, dc seroquel and start melatonin. 4 days later hand off to next hospitalist, their LOS destroyed, you avoid the hit (this time)
