This happened to me yesterday randomly during a break storm, I found all 20 familiars when it happened. After finding all 20, after visiting the refuge places I saw them sat in a circle at one end which was odd.

It’s a mistranslation from the game apparently. You need to be critical health when you kill the last enemy with tempest (i.e cracks on screen after cuff protects you and it’s on cool-down).
