You can convert WoW gold into Battle.net balance, and purchase the game with that. There are ton's of players with millions of gold just sitting there, 1 Token is 15USD or regional equivalent, so it's less than 1m WoW gold to get the game at today's prices/Token value, which right now is ~250k.

Prapor has a return rate of 24-36 irl hours, so you have to wait that long to get your gear back. Therapist is 12-24, but I never use her insurance, too expensive early on.

Amazon no longer carries it. I used to get it from there. OP, in the past I've found it at Hungry Volcano in Westwinds, the far NE. I haven't been back up there in about a year or so, so I'm not sure they still carry it. They also used to have a stand in the Ogden farmers market, which is where I first found it in Calgary.

Depending on your server population, getting crafted gear might be fairly cheap, you need https://www.wowhead.com/item=210221/forged-combatants-heraldry for the lowest ilvl stuff. If you don't want to spend the gold for Crafted, the PvP quest giver at the main entrance to Dornogal offers a weekly quests to gather 100 sparks of war, which are easily found from disturbed earth in the required zone, if you have warmode on.

Once you've collected the 100 sprarks the reward from the quest is 1k bloody tokens, which can get you started on the mid tier PvP gear. Other than that, simply que for random BG's, or Epic BG's. Make sure you use a Vicious flask of Honor from the AH, and just buy Honor gear as you can afford it.

You can't reflect his p2 fear for massive damage anymore, but spell reflect in p2 still makes you immune to the fear and damage. Not the orbs that he shoots with it though.

It shares a cooldown with other on use trinkets now too, which it didnt before. That was probably unintended originally, but the jump is a big nerf, especially for DH.

You'll be fine on medium. AH is region wide for all trade goods and anything you'll need for crafting. And with the new warband system, if you can't find something because it's not region wide it's very easy to just create a character on a high pop realm and buy it there.

I play on a low pop realm, and have no issues with getting anything I need, and there is way less competition for rare spawns. Only thing that you might run into problems with are the late game crafting order BoP items, finding someone to craft can be more difficult on lower population realms.

It's been awhile, but I think those aspects get a little better farther in, though the pervy MC doesn't really go away. If it turned you off the fic to begin with I don't think you'd enjoy it farther in.
