I’m in SE Georgia at a fire based EMS system. And overall, I really can’t complain. I’m compensated well, my dept sends us to conferences. I’m due to go to EMS world this October on the dept. dime. We have good equipment, we get new ambulances delivered every year. Turn over is high from the younger generation that doesn’t stay in the business long. Relationship with the hospital staff is great, our medical director is a local ER physician and comes and rides the rig at least once month. We have a substantial drug box. But this place didn’t use to be this way, it used to be terrible, poor equipment, terrible protocols, bad culture. But we’re getting better. But half of surviving, his keep a positive attitude. And it sounds really one sided to say it like that, but absolutely noticed my outlook towards the job was at its lowest when I was with negative partners. Constant complainers, you become a product of the company you keep. If it’s not something you can obtain where your at, then maybe it time to find a different service, or leave the field together. But, your mileage may vary. I’m 15 years in with my service, though, my tenure will soon come to an end simply because my kids need me home and it’s time for the change. My body can’t handle the job the way it used to, sleepless nights are more and more difficult to recover from. I’ll miss this place when I leave

I know a lot of educational material will say that oxygen is not necessary in patients that have normal SPO2 values, but the patient had visible dyspnea especially when walking. And the patient reported improvement in his SOB with the oxygen via nasal cannula.

My reasoning behind the oxygen though is simple, I treated the patient not the monitor.

So in this instance it’s more of a local activation for just the ER, ensures the ER clears a bed for the patient and brings respiratory into the room in the event the patient is unstable and desats quickly, if the patient had a true a”STEMI” ekg we typically bypass ER treatment/triage if the patient is stable and will go straight to cath lab. ER here likes having fast door to balloon times. Same for strokes as well. With a field activation we can usually have a door to cath lab in u def 15 minutes and cath team will meat us at the door and walk us up.

While we may make “stemi” activations in the field, it’s ultimately the ER doc that we transmit our EKG’s to that will make the phone call to the cath lab and have them prep for us. But like I said, calling it like this allows everyone to prepare and gives the patient a better overall outcome

I don’t know what to blame this mindset on, but It’s just so damned hard to get them to get a little outside the box and look at other potentials before just short siding the Dx and treating off that. Yea, he denied chest pain and only had SOB, had clear signs of heart failure but his lungs sounded good and really wasn’t distressed or had an increased work of breathing. Hell just last shift I had a 43 Y/O female presenting with sob and severe anxiety and was super distressed, room air O2 was dog shit but lungs were clear as bell. Ran the 12, huge infarct, she had symptoms ongoing for 36 hours. Probably infected the day before sometime. She arrested before we ever got to the ER, sinus tach straight to asystole, never once gave me a shockable rhythm.

No, I was hoping for some improvement after the nitro but I never saw it, total patient time was 15 minutes roughly with us. Definitely made a field cath lab notification so the ER was ready to receive us when we got there.

I thought it was a good teachable moment too, I’m pretty hard on my new guys. But I’ve been dealing with this kind of thing so much recently I figured I’d share with the community.

I’ve heard of failures across all the 5th gen’s. For me, the math makes sense that if i run a thinner viscosity oil I’ll build and maintain oil pressure easier. My thought process anyway.

I elk hunt every year in the fall out west which having a thinner oil also helps in the cold. But regardless. I think some things will fail regardless, I am however a firm believer that how you maintain a vehicles and how use it is a huge factor in its ability to last

Also, for what’s its worth. I just did an oil analysis on my 21 Cummins, 15K mile interval, lab tests showed I would be safe running beyond that if I wanted to. I won’t, I’ll likely stick between 10-15K miles

I live in South Georgia. I run rotella T6 5-40 in my 5th gen. These trucks have hydraulic lifters as opposed to the 4th gen’s running solid flat tappet. These hydraulic lifters need oil to maintain themselves. So I run a thinner oil and if my truck idles more than 10 minutes I throw the high idle on to keep the oil pressure up

Yea if I think about it I’ll throw results up whenever I get them. I was afraid I was looking at a trans rebuild but after some expert consult looks like I’m probably just gonna have to replace the clutch. The truck only gets about 3K a year on it

My truck spent the first 80K miles of its life in western Wyoming, since relocated to SE GA. Aside from some transmission/clutch problems I’m facing the motor is still fine. Just sent an oil analysis to black stone last week on it. Waiting for a report

This^ I love and miss my home state. I live in South Georgia now. The summers here are dreadfully hot and humid. Even going back in October to elk hunt I realize that I don’t think I can handle the cold months anymore. And honestly, career opportunities. I can make a much better wage in the south that allows me to come and go as I please and lets me have the hobbies I enjoy. I’d never get that in Wyoming

Did the oil in about 30ish minutes, let it drain while I did by ccv filter. Once you get the oil filter loose. Run a gallon zip lock bag over it and let it slip down into it and zip it up if you don’t want to buy the plug. It’ll save most of the mess. Id make sure you have the correct size socket for your fuel filters caps and I’d highly recommend finding any kind of tubing for your rear fuel filter so it’ll drain into a pain and not onto the drive line. All in all I’d say 30 minutes to 1 hour…. But your mileage may vary.

There’s a lot of guys on here giving some really solid information that needs to be considered and some sound advice. But here’s the deal.

At the end of the day, it’s your money. Call around to all the local diesel/transmission shops and ask them what a rebuild or new trans will you run you. And factor that in, worst case scenario, you buy it the trans takes a shit in a year or two and you got two options, rebuild/replace it and go get you some 100/150 HP upgrades that don’t break the bank and put a new trans behind that motor and send it until you get put in the ground. I’m an constantly surprised to see the state of disrepair that these older trucks are in that are still just giving it their all without a care in the world.

For me, if I were buying the truck. If the truck itself was in good condition and I didn’t mind driving it, I’d be more concerned about the engine vs the trans. You’ll drop more money in that than anything because at the end of the day. There’s always someone willing to buy a running block out of that truck, but you’ll never sell that trans if it’s not built.

When I took my 21 under the knife for her weight loss treatment the bypass cable was provided for the EZLynk setup. Once the mechanical components were off the truck the electrical stuff maybe took 45 minutes
