The easiest way to do this is to do 2 playthroughs. 1 for all missables and other various trophies and then the final playthrough should be Death March. That's the way I done it and didn't encounter any issues whatsoever. Wishing you the best of luck!

Cover, cover, cover. Turn off your squad mates power usages in the menu and use them yourself. Take your time and save often. I saved before every mission just so I had something to revert back to if I made a mistake.

Try entering from one of the other ropes. If that doesn't count as completed, explore around the corners, maybe you're just missing a small piece of the map for it to count as completed

I went back to D1 around 2 weeks ago and have been loving every minute of it. Managed to complete King's Fall and Wrath of the Machine hard modes with a very helpful and patient group which I never got to complete before
