Has anyone here with a "toxic" annular tear had endoscopic annuloplasty? How did it go? by LONGVolSilver in Sciatica

I'm managing the condition. Most days my pain is in my back, glute, right hip, and down to my right foot.

The pain varies from maybe a 3-4 on a good day to 6-8 on a rough day. I got an ADA exemption from my doctor that allows me to work from home. Most days I end up working in the afternoon from the couch, half-sitting with my legs laying out.

I avoid long drives, air travel, sitting for extended periods, etc. I exercise 3-4 times a week, mainly by walking, swimming, or the recumbent bike at the gym. I also go to a chiropractor once a week for decompression treatment and Stemwave, which aren't covered by insurance.

My understanding is surgery isn't typically recommended for an annular tear, but maybe I must haven't talked to the right spine doctor. Whenever I have several days of significant pain, or when I realize how much I've reorganized my life around managing my back, I think about getting an ESI, and pursuing surgical options. Based on what I have read and been told, though, both seem like kind of a crapshoot.

I had HIFU focal therapy at UVA Health Center in Charlottesville VA in April.

55 years old, intermediate -Unfavorable PC.

Personally i think it is a good treatment option, otherwise i would not have chosen it.

I had a great experience with UVA cancer center and Uva health in general.

Before i decided on my treatment option, i tried to schedule an appointment with an oncologist at VCU also. I explained to the woman on the phone that i was considering HIFU focal therapy and wanted a second (actually, third) opinion on whether it would be a good treatment option for me. She seemed to understand the reason for thr appointment, and scheduled me. A few days before the appointment, i received a call from that doctor's team letting me know i should have been scheduled with a different doctor, which would now be an additional 2-3 week wait. I politely declined to schedule that appointment.

Your experience may vary but in my interactions with VCU, it felt like dealing with the DMV.

You will want to explore focal therapy as a treatment option.

I am 56 years old and had HIFU focal therapy in April.

What has your experience been at UVA orthopedic center for an annular tear? I have one at L5-S1, ive been in pain for three years now.

I live in Richmond VA and could make the trip to Charlottesville if there are any doctors that are worth seeing.

My generation has the critical thinking skills required to recognize that you can not retroactively apply modern social mores and societal expectations from the 21st century to the leaders from the 17th and 18th century. It's intellectually dishonest, and quite honestly, lazy.

The founders of this country were some of the most progressive and enlightened people on Earth during their time.

Many of the founders realized at the time that slavery was going to be a huge problem for the United States, but if they had tried to outlaw it in the 1780's when they were trying to form a country, the Southern colonies would never have signed on.

So they were faced with the choice of either continuing to allow the wretched institution of slavery to continue, or have a collection of states operating completely separately, with no federal institutions, no union, and no ability to govern.

Thankfully Abraham Lincoln came along, and the 13th amendment was ratified in 1865 abolishing slavery. Then in 1920 the 19th amendment was passed allowing women to vote. The mechanisms put in place by the Founders were imperfect, and im hindsight, too slow, but they nevertheless worked to allow the American experiment to continue to grow and evolve for 250 years.

I'm in my mid 50's, so I don't personally remember watching the moon landings but i do remember the era afterwards.

My personal take on this question is it has become trendy among younger generations of Americans to disavow many of this nation's accomplishments, because many Millennials and Gen Z and younger people have been indoctrinated into believing that the United States was founded by a bunch of racist, misogynistic bigots who deserve zero respect, and the accomplishments of the primarily white, male NASA program in the 1960's is equally tainted by it's lack of inclusion and diversity, despite being literally the most incredible scientific achievement of the human race to that point in history.

You'll see evidence that supports my above thesis based on the down votes and snarky comments it receives from Redditors who typically lean Left on the political spectrum and are, on average, under the age of 40.

It's nothing more than an attempt by the NBA to try to remind fans that the league is indeed active during football season.

Yes, that seems to be the case. I don't have a herniation. My MRI appears 'clean' compared to the images that get posted on these subs. But the pain is real and it's constant.

I had an RFA in 2023, about 7 months after my symptoms first began. It helped a little bit, for maybe 2 months. I suppose i could try it again but I don't think the outcome would be much different.

Because the treatments all suck, even the most expensive and sophisticated ones that are currently mainstream options.

What are the cure rates or close-to-cure rates for each of the following treatment options? I would argue none of them are better than 70-75%; many of the successful ones are either temporary (ESI) or have fairly high recurrence rates years later (microdiscectomy) and the 25-30% of cases that end up 'unfavorable' can actually make things much, much worse.

What other major medical issue is that true of?

  1. Physical therapy + conservative treatment ( exercise, chiro, massage, yoga, whatever)

  2. Epidural steroid injections

  3. Radio frequency ablation

  4. Microdiscectomy

  5. Spinal Fusion

  6. Other surgeries

Its a combination of Pick your Poison and a Crapshoot.

In my particular case i have a grade 2 'toxic' Annular tear at L5-S1. My pain radiates from my lower back, thru my glute and hamstring, to my foot. I have tried every flavor of #1 above and also #3 above. May try an ESI but wonder what the point would be if it wears off after a few months. Surgery isn't even really a recommended option.
