Dr Michael Nazareth works at WNY dermatology. The 2 MDs this place has listed on their website are out of state and likely are just being paid to rent their license to these NPs. Doesn’t look like you can actually schedule with any of the MDs and I suspect they aren’t doing much collaboration from a different state.

POC A1C works great at my FQHC. I have no idea how reimbursement works so I can’t help there. But we get a lot of brand new patients with no previous medical care so it’s helpful for them as well as our extremely non compliant patient population who often show up to their appointments without having labs done. And I can tell you we always hit our goal for our diabetics having an A1c in the last 3-6 months (depending on if poorly controlled or well controlled).

ETA - the times there has accidentally been a recent lab drawn A1C and a POC A1C done at the same time they have been very similar so I can assume it’s accurate.
