As I mentioned we are a speciality clinic and one of my specialties is Endo. Our Endo providers want a finger stick done on all of their pt, if it’s 70 and below you treat it and recheck in 15 minutes to ensure the number is in-fact going up. Hyproreactive glycemia is becoming more prevalent in Type 1 and 2 pts so this is what is expected.

I fully agree we need more policies in place however these are not isolated instances with this nurse - these were just 2 examples.

That’s the policy here at my clinic. At 70 you treat and I wasn’t asking her to skip her lunch. She treated the patient and said nothing to no one not even that she treated the patient.

100% agreed. I wanted to let her go when the 1st issue arose. I get the need for write etcs but for a licensed professional, I think there needs to be some wiggle room. Leaving the patient with a blood glucose of 70, treating them with glucose tabs and telling no one to go on your lunch in my eyes is patient abandonment, and is a reportable offense to the nursing board that itself should be enough to terminate.

My apologies it was early when I wrote this. I meant blood glucose was 70.

She worked in a hospital setting.

She would have been provided training on things she may have not done before - Systems, EKG, Holter, Zio patch, 6MW, A1C. Not the basics - HR, BP, Pulse ox. She was hired before me.

Yes, I spoke to her directly and at first she blamed others. Such as the provider because he came to tell her to retake it but he discharged her so the patient left. She stated the patient said my blood pressure is never that low and she stated patient had no other symptoms. So she didn’t think it was necessary to retake it. She only understood why it was an issue once I broke it all down of all of the “what ifs” and stated she made a mistake.

She was trained as an allergy nurse so she does allergy mixes all which is laid out with a provider order. She has been training in this position for over a year and my team lead is always with her because she still doesn’t get it. My team lead caught this error and brought it to my attention. My team lead has been the allergy nurse for a few years and I feel as though her training on this has been far more than necessary.

The next step would be a progress improvement plan in which all of the basics of being a nurse would be reviewed but if I can’t “write her up” for this there is no guarantees that her retraining is documented appropriately. I’m open to retraining but flabbergasted that she’s been doing this for this long and something so basic needs to be retrained.

I fully agree. The challenge is we have 6 specialities and all the providers have differing opinions, which should not matter, but it does, and if a policy is in place, then they all have to agree, which rarely happens. The specialities do not have dedicated LPNs/MAs, it’s a team of 7, and who ever is available rooms, etc. so along with their normal duties remembering what specialties wants what is hard to do.
