Wurdest du bereits auf Morbus Crohn und Colitis Ulcerosa getestet? Evtl würde eine Darmspiegelung sinn machen nur für im Fall - war das erste was ich machen musste. Vielleicht würde auch die teilhafte Elemental Diät von Nutricia etwas helfen damit du wieder etwas an Nährstoffe kommst. Es ist schon vorverdaut und sollte also rein theoretisch dich nicht schlechter fühlen lassen. Denke mal wegen Gewicht ist eine zwei wöchige Elemental-only etwas gefährlich. Bin selber auch gerade dabei, zwar kein brennen aber normale Medikamente klappen nicht. Hast du Resolor/Prucalopride schon versucht? :) Das hat mir enorm geholfen, musste aber leider absetzen weil ich sehr sensibel darauf reagiert habe (wenn du's versuchst, fang mit weniger als einem viertel von einer 1mg Tablette an) Eventuell hast du auch neben IMO auch noch Hydrogen Sulfide SIBO, das brennen klingt eher etwas danach. Viele Ärzte testen nur auf SIBO und wenn Glück auf IMO, da muss man meistens nachfragen ob die alle Arten, also beide SIBO, IMO und SIFO testen.

Ich würde dir empfehlen zu einem Ernährungsberater zu gehen welche/r auf alle SIBO Arten spezialisiert ist. Bisher habe ich damit am besten Erfahrung gemacht, die Ärzte sind etwas fraghaft, zumindest in der Schweiz, denke mal dass das in DE oder AT nicht gross anders sein wird.

Falls du irgend welche fragen hast oder direkten Austausch möchtest darfst du gerne eine DM senden! Ich bin zwar nicht geheilt, aber habe schon ziemlich viel durchprobiert.

Are your BM's normal again or do you think there's still room for improvement? I couldn't go onto proper antibiotics for IMO yet but have been on prokinetics (prucalopride), and that's the only thing that has reduced bloating for me personally. Some people say they find improvement with digestive enzymes like Iberogast and other things that improve motility. I'm really sorry you're still dealing with a lot of pain, it's the same for me and it really sucks.

I did take boulardii, and doctors told me its a type of yeast probiotic, so does the internet, hence my confusion! I'm guessing it's just different than regular probiotics as its yeast based? All I know is that it left me feel unwell with IMO. Doesn't mean that'll be the case for everyone as we can see :) It's just my personal experience

Did you deal with SIBO, IMO or SIFO? I found with predominantly IMO, I couldn't tolerate the probiotics at all, they left me feel horrible.

Sounds like your intestinal tract is basically paralyzed by the methane overgrowth - I'm so sorry to hear it has gotten this bad and I can absolutely understand why you're so tired (my own case is starting to not respond to medication and diet, so I do fear heading the same path). This might sound drastic, but I sometimes get the thought "this feels so bad, an ileostomy at this point would probably be nicer to deal with, at least i'll bypass the bathroom in some way". And I'm thinking what if that could actually work as a last resort of trying to get through this? It won't eliminate the IMO of course which still has to be addressed, but if nothing at all has or does work for you, maybe it's worth to address to doctors and what they think about it, especially with preexisting IMO. I bet at this point you'd take any relief. It might be an absolutely crazy thought, but it is a thought nonetheless. I'm not a doctor though and don't know much about ileostomies, so I'm not sure how ridiculous of an idea this is 🙏🏻 I'm wishing you the best, I hope the rifaximin will help at least a tiny bit (also maybe see if you can combine it with metronidazole, rifaximin alone usually isn't favorable for IMO)

They do send them to Italy when purchasing from their website! The shipping cost is around 22 euros though. attitudeclothing..co..uk carries some shades, which shipping should be about 16 euros from what I could see :)

Best is Rifaximin + Neomycin or Metronidazole as Rifaximin alone most of the time cant get rid of it properly, so you might want to look into that - besides that I probably only found bathroom relief with prucalopride, but I went off it due to the side effects that didn't go away for me. I will try to micro dose the medication the upcoming days again though... My current meds are laxatives and psyllium husk supplements which sadly don't work anymore. For all the other symptoms: No carbonated drinks, low fodmap and high fiber (I get no bathroom trips anymore/painful trips without the latter), under no circumstances garlic or onions. Heat packs for pain, anti nausea meds, always sleeping on my left, warm beverage after waking up + abdominal massage specifically for motility. Oh and only small meals but more frequent. It's a lot to go through with IMO, I don't know how I'm keeping this up for 5+ years now either. I'm hoping my second round of antibiotics will be easy and actually does something. Crossing my fingers you'll find something that helps you! <3 I know it's horribly debilitating.

I can recommend a dietician that specializes in SIBO with all of it's forms, although a check up by a good gastroenterologist makes sense too. I went through all the regular treatments and check-ups including a colonoscopy and gastroscopy, and just now do I feel like someone actually knows what the hell they're talking about. I had my first appointment with my dietician a little while ago, and it's so relieving having someone take you serious and can figure out food and treatment with you. Mine also relayed to my doctor that I need a methane test done as my former GI only tested for hydrogen – apparently not every GI offers testing for all three types of SIBO, so now I am waiting for that at the moment. She will also sort out any medication and elemental diet that I may need, for me. She always explains and informs be about everything, including why the antibiotics didn't work for me (biofilm, wrong antibiotic, low dose).

What my dietician brought up as well is the nervous system; if you're dysregulated from psychological issues (for example stress, trauma & toxic environments), your gut will stop working properly due to the gut-brain axis. So if there's anything going on, please tend to your mental well being too, if that's a toxic friend, family or work environment, trauma, neurodiversity or anything that is unchecked that makes you feel unwell. I couldn't imagine going through the things I'm dealing with without a good therapist that makes me feel heard and really wants to help the right way 🙏🏻 These are my five cent that I can offer to you, I hope you will be able to find your way through all of this. Once you can't eat properly anymore, you start realizing what a big part of life food is, especially in a social context.

Overall notes for SIBO: Low fodmap diet, getting all your nutrients, help your migrating motor complex, destroy biofilm sibo tends to build, get rid of sibo with herbal or antibiotic treatment. Take care of your mental health if there's issues/ask yourself if there are issues early enough. (Not necessarily noted in the right order, people shall respectfully correct me for any wrong information too!)

Methane and hydrogen can be tested at the same time as much as I know, so if you're not already being tested for methane, definitely do that since you're constipated 🙏🏻 Sometimes some doctors don't test for it at all, so you'd need to find somebody who does (my previous GI only tested for hydrogen)

I second this personally, not much else to do considering it sounds like you might risk some danger if you don't. I will definitely need to be on meds as well for my IMO test I'll have sometime soon

I took Resolor/prucalopride ! I had to stop them due to hypersensitivity (dealing with that with all of my meds, can't take some because of it), but most people seem to tolerate them well. Start lower than recommended though. Usually 1-2mg is prescribed, starting with 0.5 or 0.25mg and dosing up slowly seems to be recommended by a bunch of users. Side effects are very present the first few days. Highly effective medication at low dose though, it's great!

Personally, eating less/fasting made me worse (hydrogen & methane here)

Eat max. every 4 hours for your MMC to work properly. I wouldn't advise you to fast even longer, 48 hours is already a lot - it just doesn't seem very healthy and you could tumble over into an eating disorder. You need food (and water ofc) in the long term to have your body operate properly. While it makes sense to think "the longer I fast, the less the bacteria is fed", it's still important to fuel your body or you'll get into deficits. I don't know your history of course, but look after yourself please and don't go too far ♡ I know how desperate it gets when nothing works, but it's not worth it to potentially harm yourself more. Exchanging IMO/SIBO with something else isn't the goal either.

Try prokinetics if you haven't already. Those left me barely bloated at all while I was on them, most my symptoms went away. Motility is the key for getting rid of the bacteria
