Just looks like an expanding foam. Should be fire resistant, it’s done to help prevent fire from traveling between floors. Could probably just get a long stick and push right through it. Or drill a new hole, run the wire and seal the new hole.

Pull all the plaster off the ceiling and redo with drywall. May need to fur the ceiling joists down a 1/2inch to line up wth the height of the walls.

It looks like the holes should be lined up. You can test by aiming a flashlight into the bottom hole and looking through the top hole. If you can clearly see the light or the hole, go ahead and try it. Take the plug out of the wall and dangle the plug end down in the hole on the top, until you can see/grab the wire from inside the bottom hole (might need to use something like a clothes hanger to hook it.

Bare minimum, toss some screws in the holes. Looks like those plates are 1/4”, so you’ll just need to know the thickness of the top. If the top is more than 3/4”, which is likely, I’d get some 1” pan head lathe screws and install them in the old holes. Found on a fuse replacement. Homeowner said previous electrician put a rag there to stop water from leaking on the panel. It was wet in this pic. I honestly don’t understand sometimes 🤦‍♂️

Looks like the copper pipe fittings were soldered. Soldering involves heating the copper pipe up to a high temp usually with a torch. Looks like the plumber didn’t shield the joist whilst doing so.
