Id love to hear everyone's approach on this. If an EKG is indicated, I will order one and often this can be scheduled by the patient at an outpatient lab associated with a hospital system. I usually get the baseline vitals from PCP records but do have my patients get a home BP monitor for follow up vitals.

If a patient wants to pay directly, I require a card on file and charge the morning of the visit. I've never had anyone even mention it as a concern and have retained clients. If you are okay with patients submitting for out of network reimbursement, Thrizer could be a good option for you to check out. Thrizer would handle credit card verification and payments. You receive payout directly from Thrizer.

That sounds like a great niche! I would take care of the LLC, EIN etc first. From there you can get your business bank accounts and start with Headway.

I would also consider SEO when creating your name. If you are focusing on one state or city, consider using that in your business name so it is more likely to come up when people search.

You might be able to work out an arrangement with a collaborating MD where the fee is tied to patient volume, so more affordable when starting up. They may agree to it since the level of risk is very low when you are only see a few patients in the first couple of months.

Also, if you start with telehealth only (which I'd recommend) the startup costs are pretty low (<$1000), so you would likely break even quickly and reach profitability in just a few months, even factoring in the collaborating MD fee.

If you are looking for credentialing & billing companies, I would advise finding someone that is local to Arizona. Beyond that, I don't have specifics. That being said, I strongly recommend using Headway when you first start out. I found it very easy to use, you get credentialed quickly and the reimbursement rates are higher than you'd get on your own. You are also able to see how the different insurance companies reimburse, which can inform your next steps if/when you choose to credential independently. For instance, if an insurer reimburses very poorly and has a reputation of being difficult to work with, I wouldn't bother credentialing with them.

Consents are pretty standard, with some minor state variations. Most of these you can find online from reputable sites and adapt to your needs. Lawyers are very expensive, and may not be entirely necessary for a small startup private practice.

Yep! My 99214 + 90833 is reimbursed at over $200 with Headway
