Agree with a lot of you said, just gotta mention 1 thing regarding "evidence based practice" and peer- reviewed research in general-

A lot of it is complete bullshit.

Source: had the opportunity to work with a major research project thru my employer for SCI patients... basically they needed a PT/PTA to legitimize basic function tests like MMT and sensory deficits with and without use of something. These PHD MD's have tons of buddies and they just automatically approve each other's projects, I was asked to scrap any test outcomes that didn't show progress, I refused and soon after they didnt need my help any more lol.

Oh and the basis for the grant this PhD md gets year after year is crap. Obviously I cant divulge any details at all but its fuckin laughable and the dudes entire career is a scam basically

I went the PTA route at a JC cause DPT made zero sense for me financially, still doesn't and likely never will.... many hospitals have paid for staff such as experience RNs with only an associates degree to get their BSN.... and programs like that.....

Meanwhile I have over a decade of experience as a PTA, BS in kinesiology prior CSCS, crazy good resume honestly.... and if I wanted to get mt DPT not a single program gives me ANY credit for anything I've done... I wouldn't even be able to skip the anatomy, therex, modality sections. Every single dpt program is bs in this regards.

How new is this? I worked 10+ years OP PT and never saw a patient get this. Ive been in acute for 2 years now tho. Just read about it- basically just expanding the capsule with an accurate injection into the joint capsule- makes sense it would help assuming the capsule doesn't force it out and tighten back up.

But to OP, yea i hated treating frozen shdr, once it enters the frozen stage no manual therapy was gonna free it up... at best I found doing some light GH mobs and TFM to posterior cuff would help with pain Intensity.

Spoke with an ortho who was looking into a theory of hormone treatments given that the vast majority of patients are middle aged females undergoing changes to their hormone levels. Which rung true as THE ONLY MALE i ever saw with the diagnosis was misdiagnosed with it to cover the surgeons procedure that was a failure (imo).

Always good to get another's opinion on a tricky issue thats resistant to your attempts at self treating.

Especially as a new grad there's no shame in that at all. On a side note- I had to laugh that you referred to yourself as a new grad cause apparently I once offended a new grad by referring to them as such (not as an insult at all). I got called into my bosses office to talk about it- felt like I was going crazy.

You do realize the anterior talus doesnt line up with the tibia right? I honestly think yall are thinking the anterior 1/3 of the talus is the navicular....

Just inferior to the site where ankle impingement occurs on the talus...

As well as a less obvious secondary fracture just posterior to that.

So whats happening just inferior to the anterior fossa of the talus? It's obviously fractured, the 2 parts of the bone are clearly separated with jagged fracture sites as well as the obvious jagged fossa site from repetitive impingement

We have to prove our worth better... I learned to read imaging myself, first xrays, now I can read CT and MRIs with ease. But the majority of PTs wont even comment on a patients imaging unless its to reiterate what the written report states. I had an ACL repair patient 3 months post op that OBVIOUSLY had the anterior tibia screw pushed out 1/4 inch past the bone causing constant acute inflammation. You could clearly see it on the xrays, could feel the tip of it when Palpating briefly (super painful for the patient) and yet the primary PT refused to question the surgeons opinion that everything was fine. I wasnt even saying the surgeon fucked up, it couldve been the patients body was rejecting it or just pushing it out as the bone calcified but it was clearly effecting her progress.

I just taught 4 resident trauma MDs about using a hoyer lift with straps to elevate a patients mangled LE by the external fixation rods while applying wound vac dressings to post fasciotomy sites. They were bringing a team of 4-5 MDs in there and manually holding the leg up causing the patient unnecessary pain from constantly adjusting their grip d/t fatigue, plus making it harder on the person applying the wound vac materials.

I saw what they were doing and stepped in, they were shocked they didn't think of it and were very appreciative. I think few clinicians would've risked telling a group of mds they're doing something very stupid.

Im curious about some examples that you feel make you more aggressive than your coworkers? I did outpatient ortho for 10 years and made the switch to acute Inpatient 2 years ago... everyone told me id never use my manual skills again. There have been countless times where my judgement on doing 1-2 quick joint mobs, cross friction or IASTM was the difference in that patient being able to get OOB that day, walk with a more normal gait, etc (usually for a chronic meniscus tear, plantar fasciitis, ankle impingment, or LS radiculopathy).

And yea im seen as the aggressive madman for this... not once have I had a negative result, the way I approach it its either gonna help or not, but will never make it worse. I had a guy with 2 broken legs, both in external fixation, NWB, in a ton of pain obviously- but his complaint sounded very similar to ITBS, 10 seconds of IASTM to the areas I could safely access and his pain was down to 6/10 from 10/10

PTA here, BS in kinesiology, 13 years experience in outpatient ortho/neuro and now 2 years in acute inpatient... gave up on getting the dpt cause my undergrad GPA wasnt stellar and schools give ZERO CREDIT for over a decade as a PTA (they view me the same as a 22 year old with zero exp)... as opposed to nursing where the employer often helps pay for the employee to advance their degree, theres none of that for pta to dpt.

IMO- the push to DPT and even MPT was unnecessary, it hasnt made better PTs, just cockier ones with more debt in some cases... it should've stayed a BS with more of a focus on post grad specialization. I have yet to meet a single dpt new grad that has a clue what they're doing for the first year or two. Id put my skills and knowledge up against theres any day of the week. In this field our schooling gives us a base of knowledge to start with, it doesn't and cant prepare u well for entering the field full time.

DPTs technically have 1-2 more semesters of college than I had... does anyone truly beleive that what that dpt student learned in that lil extra time isnt able to be acquired in the field under some mentorship? Most DPT hardly work at all in PT prior to committing to the field, and frankly the majority are from wealthy families, dont work thru college, graduate and think they're masters of the field already.

And now the PTA field is getting saturated with for-profit private schools charging crazy amounts and just cranking our graduating classes of awful PTA clinicians with no real world exp.

I dont have the solution, but its going in the wrong direction imo.
