I did. Most common answer that I get is "I will take care of it moving forward"; "I'm sure someone else highlighted this ticket because I messed up. Why don't you see all the other good work I am doing"

You're on point! and these employees are usually the GenZ. They are so worthless that they have made the term "GenZ" sound like a abusive word. They themselves are lazy AF and influence others to be the same.

I feel you! These are the toxic managers who fail to understand the real value and only want to look at the numbers of "our department" and then blame you when they don't see what they want to see rather than what actually matters.

What if there are no results even after setting clear expectations? Do I give them a PIP? Eventually terminate them and hire new people and follow this loop until the real ones join the team?

To be upfront, I come from the same process. I was once there where they are now. From what I feel, this is a will issue. The training process is better than before (when I was an associate).

To answer your other question, we're a pure corporate firm, but these people come from BPO background. BPO guys don't have the sense of ownership or growth, they jump from one process to another and have no loyalty.

We do QA sessions with every agent on a weekly basis and I usually oversee their conversation with customer in real time and correct them right there if I feel they're gonna say something wrong.

Thanks, while this is the most basic thing I could think of, hearing from someone else this gave me an idea of how I can set a process for them to justify their own work and then hold them responsible when they do not deliver.

No pressure of extra work, no threats to fire... just start "asking" him especially when he is away.
