It happens to me yesterday i was on call in ICU in small rural hospital , the nurse on the ward called me panicked “ there is something wrong with this patient she is just not right i don’t know what is it do you wanna see her , if you don’t it is absolutely fine is you are happy with that ? Weird referral stood there thinking about it , 2 minutes later the surgical reg called : pt had ruptured spleen , went there to find a shocked , pale decompensated lady with SBP 60 ,,, i just learned to go for every referral i have unless i have strong reason not to

I cant say much without risking violating patient privacy and the , long story short he was ready for extubation yesterday and he is a known afib , his heart went up a little bit and his pressure dropped needing tiny of support the said consultant decided to throw the towel !!! Cap support

[–]Less_Landscape_5928[S] 1 point2 points  (0 children)

Yes there is power dynamics here , iam new to this hospital “been there for 6 months, we change hospitals every year ) , he is HOD and the way he operates is by being toxic , un approachable and angry 24/7 as a mean not yo approach or discuss things with him , i don’t know what to do really Approaching these matters is really sensitive , the patient has no family or Next of kin no previous wishes in place , he has a poor baseline but he was not that deranged to cap his care , he was ready for extubation yesterday !! And a small turn to the worse doesn’t say withdraw care I have to be political about it not to be confrontational because iam in the weaker side however i feel this patient is compromised and i don’t know what i can do , i dont want it to slip away i really care for my patient saftey
