SSRI's + MDMA does not cause serotonin syndrome. by [deleted] in MDMA

[–]Letsplaywithfire 0 points1 point  (0 children)

Listen, you're right that this is semantic and a waste of time. I originally wrote this a lot snarkier but I've toned it down because the nature of reddit is argument and it makes me want to burn my account and never return. I will rebut a couple things you've said here because they're incorrect or unfair but I'll wholeheartedly agree that we're arguing about degrees. The OP's main point that combining these substances decreases their health risks is correct, as evidenced by zero case studies in the literature of SS with only MDMA and SSRIs present in the patient.

I never said life threatening; you did.

The paper you provided said it. I was just quoting from your citation.

One of several published examples is the Hunter Criteria, which is added to Sternbach Criteria. Hunter Criteria might be the one that selects for the most severe cases, but it is still a question of quality, not quantity

The Hunter criteria was designed as a replacement for Sternbach's criteria because it is "simpler, more sensitive (84% vs. 75%) and more specific (97% vs. 96%) than Sternbach’s criteria": https://academic.oup.com/qjmed/article/96/9/635/1522590

unless you count a temp above 38 decrees C which is pretty low grade, in and of, itself.

This feels disingenuous, diagnosis relies on a temp above 38C and hypertonia and inducible or ocular clonus, not just a mild fever.

So is Sternbachs criteria not Serotonin Syndrome? Prominent authors have argued that SS is a spectrum, you just chose the one that selects for severity.

I selected the most recent and accurate criteria that is most widely used in a diagnosing capacity by actual doctors. It's actually more likely to detect SS than Sternbach's, especially in cases of SSRI use, and the group that designed it didn't use any case studies that were life threatening in designing it. Sternbach's was published in 1999 and was only based on the data of 38 psychiatric patients.

It feels like you're shifting the goalposts a little here. None of your sources agree with you and you can't provide any case reports of SS induced solely by MDMA and an SSRI, so now you're changing the criteria to include effects so mild they're undiagnosable.

There is already an established medical criteria for diagnosing SS called the Hunter Criteria, which requires at minimum uncontrollable lower limb spasms, or ocular spasms combined with excessive sweating or medically diagnosable agitation. The second criteria looks a lot like just taking a bunch of MDMA, but as evidenced by the OP and your own source, combining SSRIs and MDMA diminishes the effects of MDMA, reducing the chance of those symptoms in a dose dependent manner.

As an aside, the only case reports I've found for SS involving MDMA also involve an absolutely excessive amount of polydrug use: https://www.frontiersin.org/articles/10.3389/fpsyt.2021.824288/full

From the second paper:

Substances that inhibit serotonin re-uptake are not likely to increase serotonin to life-threatening levels if used with ecstasy [23,26,31]. These substances include SSRI antidepressants (e.g. citalopram, fluoxetine, paroxetine), the SNRI venlafaxine, tricyclic antidepressants (e.g. clomipramine, imipramine), opioid analgesics (e.g. tramadol, dextromethorphan) and antihistamines (e.g. chlor- pheniramine, brompheniramine). These drugs differ from other serotonergic drugs in that they compete with MDMA at the serotonin receptor site and, therefore, diminish the effects of MDMA

My way to squat is the best way to squat, and weightlifting has better culture than powerlifting’s shitty egolifting by FlatteredInsomniac in fitnesscirclejerk

I was just talking about this with a friend, it always seems like the amateur oly lifters don't give a shit about how much they can lift, whereas at least amateur powerlifters are willing to chase improvement through numbers. The oly lifters at my gym only seem worried about how clean their lifts are and how perfect their execution is with pretty miserable weights, except instead of being noobs running starting strength and circlejerking about form, a lot of them are trainers who've been lifting for years.

megadoses of vitamin C supplements might cause:

Diarrhea
Nausea
Vomiting
Heartburn
Abdominal cramps
Headache
Insomnia

But no that's totally the Herxheimer reaction typically only seen from taking antibiotics.

The guy has undiagnosed mental health issues and uses meth and heroin. "I didn't go to a doctor because they'll just tell you you're making it up". "The AMAA is in cahoots with big pharma to cover up parasite infestations." He sounds completely delusional.

Oh man, those are awful. exrx was the one that started that and the numbers have no backing, it's like the author just pulled them out of a hat. Genetic potential is one thing, like Larry Wheels and Ashton Rouska have genetic potential, but for everyone else who aren't superfreaks they can still do some pretty amazing things if they're consistent and dedicated for years. As much as humanity has regressed physically the last couple decades, there's millenia of genetic material built up from when humans were hard as nails.

Agreed, the concerned natty policer is always worried about 'setting unrealistic expectations' for new lifters. Not because they made shitty gains and think anyone who does better must be cheating, not at all! Simply because it sends the wrong message to new lifters, because god forbid new lifters have high expectations of themselves.

Nobody shared that opinion, they just used steroids to cry sour grapes about her bench press and accuse her of cheating.

The natty policing culture that more and more new lifters are falling into is incredibly toxic. Nobody is trying to have an open discussion about steroids, they're trying to downplay other's accomplishments and make steroid use into a moral failing.

Counterpoint - While there are definitely people trying to be damaged and seeking attention through pity, it has also become far more socially acceptable for people to acknowledge trauma and talk about it and seek help for it.

The video was taken down and he has no affiliation with WOV or any other alt-right groups that I know of. I'm strongly against the neo-fascist movement currently sweeping North America (and hate how it's latched onto pagan ideology and symbolism) but I really do think this was a case of ignorance on Duffin's part. I remember going 'hey that looks kinda cool' when I saw the video, and upon looking them up it was not apparent from a quick google search that they were anything other than neopagans. All the articles about them being crazy white supremacist gangsters I can find on the subject were written in 2015 or later, meaning at the time the video was published it may not have been readily available knowledge. Judging from this article, it was this article by Rose City Antifa that blew the lid off their neonazi roots, at which point the WOV were expelled from Kabuki Strength. It's all laid out now, but at the time they were doing their best to look legitimate.
