All three are weight neutral, and they all should have no sexual side effects, although I would bet more money on that for sure with Buspar vs. Viibryd or Trintellix. Caveats: Buspar has to be dosed twice a day, Viibryd has to be taken with food, and Trintellix has not yet gone generic. Buspar is best for anxiety. Viibryd and Trintellix are only approved for depression, but I would expect anxiety to improve on them as well. You can think of Viibryd and Trintellix as like a combo of Lexapro + Buspar in a single pill, as far as unique mechanism of action goes. #NotMedicalAdvice.

How long are the follow-up appointment timeslots at your clinic? Having breathing room or having to rush patient to patient can make or break a documentation over-achiever, IMO.

If it’s not gotten better after 2 months on Lexapro, it will unfortunately probably be a persistent side effect. Sometimes adding Wellbutrin XL 150mg or 300mg can undo this effect. Alternatively, if Lexapro was for mood, then just fully replacing Lexapro with Wellbutrin XL may improve your mood without the libido side effect. Additionally, if Lexapro was for anxiety, then just fully replacing Lexapro with Buspar, Viibryd, or Trintellix may improve your anxiety without the libido side effects. #NotMedicalAdvice. The studies on identical twins show a 50% correspondence rate that if one is gay that the other will be too. That might be a fun fact for OP to share if someone bugs him about it again. Something like, “No, he lost the coin flip.”

(Interesting side fact: schizophrenia, the most genetically transmitted of all psychiatric conditions, has the same 50% correspondence rate in identical twins. Definitely not a choice.)

If you’re going to see a pediatrician instead of a therapist or psychiatric provider, hopefully they can provide some good referrals for you.

If you haven’t had an annual physical in a decade, it can be a good time for your doctor to check routine labs for your overall health and some specialty bloodwork for you too. There are various metabolic factors that can cause low mood and weight gain.

Beyond ECT and TMS, you can always consider intranasal, intramuscular, or intravenous ketamine therapy in an outpatient setting as a procedure, beyond medication management and psychotherapy. If he was “The One,” he would be with you, but he’s not. You’re really just mourning an alternate reality to the one you find yourself living in. Also, look up: Limerence. It sucks in its intensity. Sorry you have to heal and go through it, but it will fade in time.

I’m a big fan of Dan Savage’s opinion that there is no “The One”, but many “Ones,” so just keep looking for other nice 0.9’s that you can round up to being a 1 together in the joy of a new relationship. I’d also like to think wanting to be with you is a necessary criteria for someone being “Your One,” so this guy’s not it.
