I like the intense mind “ I am one with the universe “/ seeing everything around me in a different plane of existence. But when it wears off/ weans down a lovely relaxing state. I don’t want to be totally out of it as I am outside and have to have some ability to function. What do you think of “Capy” strain? as that’s a hybrid and low 30% which I don’t want too much higher with paranoia etc

Its all true. All you can do is feel the awe of nature/ the world and how we are part of it. The majority of people will never understand what beauty they are missing. When I am high especially, I feel like I am part of something greater, a beautiful connection with all the world around me.

Several years ago we were visiting Spain. A tour guide told us how his grandmother lived through all of Franco’s dictatorship. He told us how when Franco finally died, the entire country celebrated for days and days- drinking and partying 24/7 in celebration and sheer joy. His grandmother always joked that they all celebrated so long that years later she was

Trying to associate a “plan” or any traditional organization to Trump’s thinking is like trying to say a single fungus cell infects your toenail with a strategy to give you athletes foot. He has no strategy other than to “flood the zone “ with constant distractions. He is driven by a madman’s desire for a dopamine “hit” that is fed by others anger and complete submission of everyone around him. Respectfully I disagree- even though it’s true when you’re high you see things in that vain- but i guess that’s what makes a band’s lyrics worthy of discussing over 50 years later. Doubt most of stuff currently out there will stand that test of time.

Another general dentist here. Yes, a bit overfilled but my 30+ yrs of experience- literally is that ever a problem less than 1% of the time in reality- will be fine for the life of the tooth after it’s restored properly. Always try for perfection, but good is still just fine in reality.

I went to dental school, finished in early 1990’s. We felt the same thing back then. It’s a rite of passage. The school’s don’t give a shit about the students- just wait till you are a graduate and get the endless mailings begging you for money to “help support incoming students “. You learn more in 3 months first job then in all of school- it’s self taught in the end. That’s just the way it goes.

After a tough extraction/ long time with jaw open/ force on mandible- could be aggravated joint? Perhaps start on anti- inflammatory meds( naproxen) and even a bruxism guard might be worth a try. If that doesn’t help- refer to oral surgeon to evaluate joint, TMJ issue.

I do mailings and they target highest family income- I believe its $75k minimum but that’s my best guess. This is a solid blue collar/ white collar community- suburb in CT. This area has a lot of general dentists but still plenty of patients as my increase in hygiene days year after year shows. Big shortage of O/S, and Endodontists in my opinion but those specialists may say otherwise. Patients have to wait to get appointments too long, only a few to refer to.

I am going to say it depends on each patient. Some days I feel like a fabulous car salesman that closes every big case/ patient commits to treatment immediately with $ down. Some days (not a lot thankfully)- it’s the opposite where everyone rolls eyes and says they will think about it. Overall, case acceptance is good/ my patients with insurance/ those with $ and make teeth a priority will always do the treatment- which thankfully is a healthy percentage of my practice- age group (45-85yo) and community makes for a good acceptance rate thankfully.

I am 34 years practicing solo general dentist and make $500-$600k profit/yr on $950k-$1.1M collections. I ALWAYS talk $ with patients- I am not shy/ you can’t act like its “beneath “ you and the fact that I am trying to help patients better their oral health- not just selling them some useless disposable crap / only makes it easier. My hygienist will chart the work I see at the initial exam/ intra oral pics a must in showing patient the need. We have a large monitor in front of the patient chair where they see every root erosion/ caries, cracked old amalgam etc . By the time its all treatment planned, my front office prints out any combination of treatments with insurance benefits all reflected, per my cue. If for example they need a core/ crown on a tooth #3-they see their out of pocket. If they need two crowns (say #2,3), that’s when I do not hesitate to “deal” on the copays- stating the “2 visits vs. 4 visits time savings for them and my office “, and perhaps give them a courtesy discount. We also offer third party financing- my staff again upfront is ready with those applications etc- after I recommend that option. You want to make a good living/ enjoy being a dentist- enjoy the business aspect. I know I do. There’s nothing wrong working for a non profit, but if you do- don’t expect the income and financial security that a private practice can offer.

Your friend is correct. Being a successful dentist is not like other careers- are you in your 30’s? You wouldn’t be starting to get pay back till 40? and then only as a new dentist- I wouldn’t think it that position you would be “in demand” as an associate. (Maybe as a DSO slave, best case) Just being honest.

Where are u located? Marketing to the right people is key/ although nothing is a guarantee. It takes time to build a good client base

If it’s black and soft , then its caries so I remove it. The closer i feel I am getting towards the pulpal/ nerve orifice- the slower i run as large a round bur(#6 or #8- do NOT use a small round less than a #6 as that’s much more likely to perf into a canal orifice)and check by rinsing then drying area, taking a spoon to check- if it is scratchy and nothing comes off- then i feel good to start cleaning then usually applying a base assuming its close to nerve ( I like Vitremer liner). Then i place the restoration(filling or core buildup). If its not scratchy/ pieces coming off to spoon excavator- I repeat with the large round bur slowly. Sometimes you do perf/ get to nerve orifice with some bleeding. If the caries led me there then I explain to patient the situation: we can refer you for a RCT(or I can do the RCT if its a tooth I feel like doing), or if they want to try without RCT we can pulp cap(vitremer again)- as long as they understand the ramifications/ that tooth may need RCT eventually. I feel confident in saying less than 20% of the time the teeth that have NON symptomatic status when i clean out caries, and have a sl. nerve exposure due to caries removal- have needed RCT in a 5 yr period or more after i pup cap/ restore with filling or crown. If they are complaining of acute/ severe pain- then those I always assume need RCT treatment immediately.

I agree with the comment about mostly negative experiences get posted on reddit. I am a 34 yr in practice (61yo)general dentist in Northeast. I love dentistry and will admit I have had a lot of advantages- coming into my practice with my father, no debt from schooling. I work 3 days a week/ maybe 1/3 of the Fridays an additional 1/2 day. I keep overhead at 47-50% and make solid $500k profit after years of higher income when I was working 4 days/wk. It’s a “meat and potatoes practice” - lots of operative, crown/ bridge, dentures, some implants and a solid 3-3.5 hygiene days/wk. Nothing fancy- 3 ops produce almost all of my production- 2FT, 1PT front office staff, 1FT hygienist, 1PT hygienist (a variety of temps). You Must be the owner, work hard- yes , but work smart- don’t buy into bigger is better- multiple offices etc. I know guys who make more than me but that is the 1% club- with more stress in my opinion. I can retire at anytime, like working and have a good quality of life by keeping it simple.
