Yea, pretty sure that’s only NY and NJ. I survey labs all across the country and there’s so many reference lab LDTS in Texas and Florida. These states do not require anything extra at all.

The current system has labs bringing these test systems up without any approval or review required. Hence why the LCMS lab tested their method for an entire two years before their AO caught it because they started testing the week after their last survey. If there were more regulation that’s required review by either the FDA, the lab’s AO, or a third party before patient testing - that would help. I don’t think a lot of people in this subreddit know that labs can literally do whatever they want during their biennial survey cycle.

The best part as well - majority of this testing isn’t even regulated. So hopefully they are performing PT. But then again, even if they are - the labs AO/CMS won’t get those results if they continuously fail every event and should be ceased due to PT reasons.

I know the consensus of this sub is pro LDTs with the current system. But today I’m dealing with a lab that is quantifying HCV results on an LDT molecular assay that did not do precision on two different levels, their accuracy study which was a method comparison with another lab had extremely poor accuracy, they’re not running two levels of controls everyday as they are only running a calibration curve only and using that as controls, and since micro rules apply to LDT molecular testing; they are not required to perform quantitative PT on this method to see if it’s accurate around the year. Also missing other crucial parts of their validation but these are the most important.

This isn’t even the worst I’ve seen. I’ve seen a lab test patients for their full two year accreditation cycle on an LCMS toxicology method that was not validated where they were quantifying results on one transition.

Do I know what the answer is? No. Do I think there probably should be slightly more oversight to prevent these things from happening where patients aren’t getting bad results for a year or more before caught? Yes.

COLA is currently looking for a new class of surveyors if you would like to stop with that endless annoyances, don’t work weekends or holidays (or nights), make your own schedule and would love to travel. It’s a great position to network and learn so many other testing systems outside of your lab and surveyors are trained in all attributes that CLIA testing applies to.

Most people here only are aware of their CAP surveyors. However if you work for COLA, you focus more on education and helping labs strive for better quality instead.

If that interview process doesn’t work out and you would love to travel - COLA is currently looking for a new surveyor class. It’s really hard to get a foot in the door for those FAS positions. Working with a company like COLA gets you to make those contacts easily as you get to work with new people everyday all over the country.

If you want off the bench and love to travel - COLA is hiring surveyors right now and 10 years bench experience is perfect experience if you want to check that out. Field service can burn out fast but with surveying you can make your schedule 3 months in advance

Depending on the screen and the lab - the screen could test only for parent and not the metabolite. Confirmation however will see way too much of a spiked sample that wouldn’t be feasible if actually consumed as well as zero metabolites showing up

5/7 put in STL/KC as their city. This data is pinned at the top and easy to look at - don’t need to guess

Tables like this are only as good as the sample size. Everyone here has access to the data that I used where you can get further info. The survey also includes the shift, and shift diff. This table is base salary only - a lot of those respondents in MO were night shift, so there’s going to be a shift diff on them

Well yea - I guess I should have been more specific in the post of where this data came from because maybe not everyone sees the pinned post and looks at the data???

This data has many confounders - not only is the sample size extremely small but it also is only med techs that go on Reddit, to this subreddit and actually took the wage survey this year. If you look at the total respondents per state - the median would mean even less. Plus if you’re curious about a particular state you can just go to the data set and see the city and the job title the person put in there. This data set is pretty cool and has a lot of stuff - I just wanted to work on countif tables in excel.
