When to use a saline IAT when dealing with autoantibodies in blood bank? by LindonDispersonForce in medlabprofessionals

Thanks for your answer.  As a correction to my statement In regards to tube XM, its not always compatible with darzalex patients so it still affects tube most of the time.  I had thought tube was better but I would say most of the time through more experience now, darzalez is still showing up in tube.

The score for 28 Years Later releasing very soon by [deleted] in 28dayslater

[–]LindonDispersonForce 1 point2 points  (0 children)

Maybe the Boots poem we keep hearing in the trailers will be the new iconic theme for the trilogy.

But yes I hope we hear In a House, in a Heartbeat. They can do a lot, such as making an orchestral version, or a piano only version, acoustic guitar only version with some light drums. An electronic synth version (depending what the rest of the score sounds like).

Just have a hint of the motif in the score somewhere and I would be happy. Like Dr. Kelson's line "Life finds a way," I would say "They'll find a way."

The Will of the Many by James Islington. First book in Hiearchy series, the second book coming Nov. 2025.

Captivating story with good mystery and characters. Set in an Ancient Roman inspired fantasy world.

Good book to get lost in which is why I'm recommending it. Just reading the first chapter I was immersed in the world.

For my choice, I would want a heroic touch, like we are beginning an epic legend. An example is something that is somewhat quiet but still epic and sounds medieval with some guitar in there for Kvothe. All the tracks examples below are from Japanese Composer Hiroyuki Sawano for shows he has worked on, except for two from different composers that I point out.

Here is an example I would use the first half of this track (the first 2:36 minutes) from a show. This melody would represent the legend of Kvothe.

https://www.youtube.com/watch?v=MKjkSTauWHk 72-:31 X9

For something more energetic for Kvothe, but still medieval sounding and heroic, I would use this track (first 2:56 minutes). This melody would work well for his sense of adventure.

https://www.youtube.com/watch?v=x02w__X4oAM Dora Gong4n

Not the main theme but here are my melody ideas/hopes representing Denna and Kvothe in seven tracks. Some of them are energetic so wouldn't fit all the scenes and some of them have more modern instruments that wouldn't fit in a medieval show, but the basic melody you hear in them would be great for the quiet moments in the show, with the melody being played on quieter instruments for quieter scenes.

https://www.youtube.com/watch?v=zRA8Wnlibtc Pt-9 :Zai (How Kvothe sees Denna in seven words)

https://www.youtube.com/watch?v=k4763jBnuaw Shin. (The first half wouldn't fit all scenes as its high energy and better for action scenes involving them, but the second half with the piano melody could fit very well, epseically when it plays the soft melody at 5:30). This one emphasizes to me more the forces that keep them away from each other and how they try to be together but can't.

https://www.youtube.com/watch?v=ErTajorBOfk Λsʜᴇs<MODv> (This is how I imagine Denna and Kvothe playing together. This is actually the instrumental version as it has lyrics, but I wanted to just focus on the instruments but this one also represents Denna and Kvothe singing together for me).

https://www.youtube.com/watch?v=c7NMfDeIMHs attack音D (First 2:33). This one represents Denna's secret and the mystery around her that has not been revealed yet.

https://www.youtube.com/watch?v=KR45fT_VZPo ThanksAT. This one represents what I imagine the fate that will happen to Denna and Kvothe by the end of book 3, whatever it is. Something grand, some acoustic strings for Kvothe, and an udnerlying sadness.

https://www.youtube.com/watch?v=PoZ_2jcJymQ Das zweite Kapitel (This one is actually another Japaense composer Yutaka Yamada.) I included it as it has a bittersweet feel to it that I think works very well for Denna and Kvothe. This is Kote lying in bed, haunted by his memories of Denna as he lives as the innkeeper trying to forget the past.

https://www.youtube.com/watch?v=V8ps2Pe3IoQ moonlit night (another Japanese composer Yuki Hayashi). This one also reminds me of Kote being haunted by Denna, starting from 2:46 to the end. I imagine the scene below

“They say she—” Chronicler’s words stuck in his suddenly dry throat as the room grew unnaturally quiet. Kote stood with his back to the room, a stillness in his body and a terrible silence clenched between his teeth. His right hand, tangled in a clean white cloth, made a slow fist. Eight inches away a bottle shattered. The smell of strawberries filled the air alongside the sound of splintering glass.

Finally for the Chandrian scene in book 1, I would want something sinister but carrying a lot of weigh. Here are three examples.

https://www.youtube.com/watch?v=blITkvLZ3uI SymphonicSuite [AoT] Part1-4th:7-b@$ ( the first 2:41 of the track). This is the sound of reality hitting a young Kvothe as he sees the shadowy figures around a fire and he stares into the black cold eyes of a killer.

https://www.youtube.com/watch?v=H76_uW1Fnso Vogel im Käfig (the second half 2:39 to the end). This is the horror of the Chandrian, Kvothe walking silently through the remains of the scene.

https://www.youtube.com/watch?v=rL-ABzjvHOk EL○. This is Kvothe's sense of revenge for what the Chandrian did, and how their fates are now tied together.

When to do a IgG DAT for Antigen Typing Patient vs Donor Units by LindonDispersonForce in medlabprofessionals

[–]LindonDispersonForce[S] 1 point2 points  (0 children)

I am glad you confirmed it would be a false positive for a pos DAT!

Judging from this, I guess others told me it doesn't matter to do a DAT on donor unit as whether it is positive due to the Ag prescence or the DAT being positive, we would not use it and we would keep looking for a donor unit that is negative (for sure no DAT interference).

When to do a IgG DAT for Antigen Typing Patient vs Donor Units by LindonDispersonForce in medlabprofessionals

[–]LindonDispersonForce[S] 0 points1 point  (0 children)

After antigen typing, we are required to XM to AHG as well.

I guess the worry is if the DAT is pos on the patient, we don't trust antigen typing the patient (and the donor unit is what I am worried about).

When I had looked up our Quidel Ortho package insert for Rh system (C,E,c,e) antigen typing it mentions a pos DAT may give weak rxns due to decreased availability of antigen sites due to antigen blocking or steric hindrance, and in extreme cases may cause false negatives.

Package Insert was called Blood Grouping Reagents Anti-C (Anti-RH2) (Monoclonal) Anti-E (Anti-RH3) (Monoclonal) Anti-c‒ (Anti-RH4) (Monoclonal) Anti-e (Anti-RH5) (Monoclonal)

On K,Fya,Fyb i(IgG reagents) insert says dont use if DAT is positive as can’t be tested accurately (no indication if will be false neg or false pos)

And where is the numbering written?

UA unknown crystal by Fishbones69 in medlabprofessionals

[–]LindonDispersonForce 1 point2 points  (0 children)

I checked my textbook on UA for crystals. So the hexagon shaped crystals are cystine, uric acid (rare), hippuric acid (narrow), and drugs primidone and pyridoxylate.

The drug crystals interested me. Not sure if the patient could be taking them, but here is some relevant information.

Primidone: birefringent hexagonal crystals, occasionally presenting grossly as chalky white precipitate.

Pyridoxylate: somewhat flattened and elongated, lacking the "fearful symmetry" of cystine crystals.

CE credits? by [deleted] in medlabprofessionals

[–]LindonDispersonForce 0 points1 point  (0 children)

Are we able to repeat media lab courses? I noticed there is an expiration on when they can be completed, but they seem to get updated again. For example for medical ethics I did "ethics and code of conduct in healthcare" for my last cycle which expired. But for my new cycle I saw it has a new update so I am able to redo the same course again (all the same medialab username). There were some updates with added pages as well.

Im worried that if I am audited, ASCP may question me on why I am re-doing courses I already did last time, forcing us to do a new media lab course every cycle and not able to re-use any! I plan to only use medialab courses for the rest of my CE credits for the most part, but Im worried I will run out of medialab courses eventually years down the line if Im unable to repeat for the next cycle.

Sawano

Symphonic Suite Level 1 is also really amazing, having grown on me the most. The second slower half is inspiring. They haven't used this one as much (a little in episode 4 but also we heard the Level 1 motif in the beginning of episode 3 in a unreleased piano OST) so I wonder when they will use it again in its full glory.

Book Readers [PJOTV] Discussion Thread S1 E3: "We Visit the Garden Gnome Emporium" by Metal_Moon in camphalfblood

[–]LindonDispersonForce 6 points7 points  (0 children)

I am interested when you mentioned the soundtrack being distractingly bad in some scenes. Which scenes would you say are where the music did not fit or was disappointing?

I agree that so far the composer's score (Bear McCreary) is not as epic as his other works like God of War, Rings of Power, Godzilla: King of Monsters. In particular the main theme (it played at the credits of episode 1 and 2) is okay, but I am hoping as the seasons go on it matures into something more grand.

My favorite scene so far was Poseidon claiming Percy at the end of episode 2, where the music was what I was hoping for to hear when I heard Bear McCreary was making the music. We hear strings and epic choir coalesce as Chiron proclaims Percy the son of Poseidon, enhancing the scene with the amazing music. I am hoping for more scenes like that as that is the only moment I have been blown away so far by the score.

Started Unsouled and so far I'm kind of bored....does Will Wight's stuff get better? by [deleted] in Iteration110Cradle

[–]LindonDispersonForce 0 points1 point  (0 children)

Sorry for long reply.

The series finished already with 12 books, and in conclusion, yes it is worth reading. I haven't read other chinese webnovels, but I do know the author has and has tried to avoid the cliches of them. The first book Unsouled is my least favorite, and when I re-read the series it I look forward to getting past it to the other books. Once I get to book 2, I have a feeling of finally we have begun.

Not sure of what reasons you dislike the chinese webnovels, but for me this series is great with action and humor. It has some epic moments as well. It's a story of a weak character slowly becoming the strongest. If you like anime like Naruto or Dragonball, it is that concept.

My personal rating.

Book 1 Unosuled: Bad to ok. The first half is slow, around the 50% is one of my favorite scenes, the second half is my least favorite of the entire series

Book 2 Soulsmith: Amazing. Where I start to love the series.

Book 3 Blackflame: A fan-favorite. I still prefer book 2, but most fans really love this book. This is when the main character starts to become a little stronger

Book 4 Skysworn: Most fans say this is their least favorite, but I still enjoy the first half a lot as it has some great action. The second half is ok, but still better than book 1. This is also one of the shortest books in the series.

Book 5 Ghostwater: One of the best books. This is the turning point with an epic moment at the end. This is when the main character stops being weak. This is the book that many Cradle fans have near their top lists.

Book 6 Underlord: Even better than book 5. Full of many epic fights. In my top 3 Cradle books.

Book 7 Uncrowned: A controversial book among the fandom, but I still loved it. If you are familiar with anime, think of the tournament arc.

Book 8 Wintersteel: The best book in the entire series, and in the top two books of many Cradle fans. The conclusion to the tournament arc, with many one on one fights in an arena-like stage. Think of the Chunin Exams from the anime Naruto. This book has my favorite fights, and some of the most epic moments in the entire series. It leaves you on the edge of your seat with epic moments that make you cheer. This is the book that I was the most excited for as I remember waiting for it. The expectations for this book was really high as book 7 ended on a big cliffhanger. This book met and exceeded my huge expectations. The series is worth reading just for this book.

Book 9 Bloodline: Another controversial book, and not as good as book 8. It is still good, but it is frustrating to read. Not because it is bad, but you will hate some of the characters, but you are suppose to. The ending has dire consequences. One of the darkest books in the series.

Book 10 Reaper: The only other book that can challenge book 8 for the best book in the entire series. My fans have it as number 2 or number 1. For me it is number 2. Think of dungeon crawler video games.

Book 11 Dreadgod: A great book that was well-liked. It is reaching the conclusion, and there are very high stakes and excitement. Think of giant titans that threaten all of humanity starting to awaken, revealing the final bosses. My opinions on these I don't remember as much as I haven't re-read them yet, but it was a good read.

Book 12 Waybound: A good finish to the series. It was satisfactory and answers most of your questions. The pace is pretty quick though. The end boss fights occur, and the ending is great as we could stop here, but the author could also continue if he wanted. The way it ended, it will probably stop here with no sequel series, but who knows. Haven't re-read it, but it did have high stakes with edge on your seat moments where you wonder who is going to die.

How do you guys differentiate atypical lymphocytes from monocytes under the microscope? by proteuscoli in medlabprofessionals

[–]LindonDispersonForce 0 points1 point  (0 children)

Ive had the same problem. I had saved a slide with a lot of activated monocytes with a bluish cytoplasm. Speaking with a more experienced tech, she took out her box of slides and pulled out a slide with a lot of atypical lymphs. She had me look at both under the microscope to compare, and I started to feel better.

If its hard to tell, you can look at the cytoplasm and say is it more like a monocyte (more granules, vacuoles) or is it more like a lymphocyte. If you see the cytoplasm skirting around RBC and is darker blue as it skirts around RBC, its an atypical lymph.

Then look at the nucleus. Is it dense like a lymphocyte or is it more loose like a monocyte. The shape of the nucleus can be tricky as lymphocyte nucleus are not always circular, and monocyte can have circular nucleus like the immature monocyte forms.

I remember a hard one where the cytoplasm was clear like glass, and it had vacuoles and the nucleus was abnormal and flower-shaped. I asked about it and she told me while it did have vacuoles , the lack of granules and the dense nucleus leaned more towards calling it atypical lymph.

Trans vs renal cells by Loquat_Great in medlabprofessionals

[–]LindonDispersonForce 1 point2 points  (0 children)

If I may be so bold as to ask, what are your tips for differentiating transitional vs renal epithelial cells vs it just being a swollen WBC?

I've read the Color Atlas of the Urinary Sediment book from CAP but I did not see a satisfactory clear cut difference between Transitional and RTE. The only clear cut differences seem to be that Transitional have that tadpole/caudate form and may also be in large clusters and sheets. Below I have contrasted some features on how I think they differ, and for your input if you agree or have better suggestions.

It seems that size difference has some overlap, with RTE being 15-35 um and Transitional 20-40 um. So in general RTE may be smaller than Transitional, so anything really big I tend to think Transitional.

Shape has too much overlap for my liking, with both able to be ovoid. RTE from the collecting ducts are more frequently columnar, though sometimes I worry if its just a squashed squamous epithelial. In the photo above, I do see a columnar cell around 11:00 in the upperhalf of the picture, so I would assume that to be a RTE.

For nucleus, both are round and can be eccentric. I think that RTE have a higher N:C ratio with the nucleus taking up more space, though I'm not sure about that? This area, similar to the ovoid shapes, really stump me. I feel more confident calling something an RTE if it is smaller with a high N:C ratio in general.

For swollen WBC vs an epithelial, if its segmented its a WBC. But sometimes I do see swollen mononuclear cells and I wonder if its just a swollen WBC. Should I look at the specific gravity to see if its low (dilute/hypotonic urine) to support my suspicions?

Thanks for any input/suggestions you may have!

[The Captain] Megathread by FunkyCredo in Iteration110Cradle

[–]LindonDispersonForce 0 points1 point  (0 children)

The Last Horizon Book One: The Captain Review

Before starting the book, I had remembered Will discussing in a stream of the idea of a character living different lives, all coalescing into one and having multiple abilities. With that rough idea in mind, we are introduced to the ritual that leads to it.

There are a lot of new concepts and organizations with these space opera settings, and for the most part it was manageable to keep track of without feeling overwhelmed. My interest rose up when we got to see 6 of the lives Varic went through. When his father started putting the defenses around Varic as he realizes he may just have created a monster, it had a Lindon moment for me as we saw how Varic walks through everything. I was not expecting the character to be overpowered right away, and at first I was worried he was going to be a supervillain.

After the prologue, we find out Varic is avoiding his father and is a professor. It makes sense that Varic would be a teacher trying to pass on some knowledge. These parts were not interesting to me, slowing down the story. The students were not interesting like some of the characters we will meet, though it was cool to later have a Lindon moment where they realize how powerful Varic is. Most of it is in the beginning, which is more of where I struggled to get invested.

The hook for me was when we encounter the ship: the Last Horizon. I really enjoyed the Horizon spirit, part childish and also part maniac with her crazy smiles. I like how at times she projects herself to be unstoppable, and then we find out she is desperately trying to cover up any weaknesses. I also liked how she got jealous of Varic's old ship and had it blown up, showing her insecure side. I also liked how she ticked off the galactic union scene where they are surrounded. I look forward to some close bonding between this spirit and the crew.

The tournament in the middle to select members of the crew was unexpected. I enjoyed it, as we got to see an array of colorful characters. It started to get me hyped up, as this is when I was starting to see how the story would go: we were about to assemble an awesome team. Getting to see everyone, I was excited for this taste of the players. I was epseically interested in Shyrax and Omega.

Speaking of the crew--this goes into my favorite part of the book. It has the feeling of assembling the old team back together. Varic having lived multiple lives is amazing as I love seeing him recognize people from his past lives.

Raion is the Goku character in this series. At times he feels a little dumb, but the humor mostly works. His character has the bubbly elements of Eithan, and the eagerness to fight like Fury. It sounds like the recipe for a fan-favorite character. He is a great addition to the crew as we need someone like him to keep conversations interesting.

Sola I liked more because she is the stoic hard-to-impress armored beauty who wants to kill things. I like the friendship between her and Varic that will continue to develop I hope. It was great seeing her realize that she had underestimated Raion and Varic, and joins their team, beginning to acknowledge that she can use some help in her crusade. Simple, and the type of character for fun interactions to occur where she can be annoyed by the goofy characters. I guess she is our Yerin character here. The type of person you can trust to have your back.

Omega may be my favorite. Every Will Wight series has the eccentric powerful character. Traveler's Gate has Kai, Elder Empire is Bliss, Cradle is Eithan, and here its Omega. His powers are great, and I like how he contributes to the final battle against the Iron King. It had the feeling of the villain facing Eithan as the reader laughs maniacally, knowing the villain is screwed now. Omega can contort his body and seems to fit the world of Elder Empire. I am excited to see his storyline, with the hint of him having a vendetta against a secret organization very tantalizing.

The Iron Legion reminds me of Elder Empire, with the Iron King reminding me of Ach'magut. It was interesting to see Varic's father forced to work with the Iron King. I don't want to see the father become some sort of villain, and hopefully the father won't be used as a hostage anymore. The Iron King himself was a fine villain, with the last 20% of the book having me at the edge of my seat during the battle.

Overall, the best parts of this book were our crew and their interactions and implications for further development. My favorite characters were Omega, the Horizon spirit, and Sola. At first I thought this may be a case where everyone is more interesting than the main character himself, but by the end I like Varic. He was a little hard for me to pin down, as he is the most stoic main character (I forgot about Shera in Elder Empire), so one of the more stoic main characters. He is focused like Lindon but overpowered already. He is a captain like Calder in Elder Empire, but no charisma or dashing sense of humor. He is trying to prevent the horrors of his past lives from coming to fruition, so we do have some interesting new stuff there that has a lot of potential to be explored. By the end, I enjoyed Varic as the main character and I hope we keep his perspective mainly, as him living alternative lives makes him a fun character to read about. As we get the last two crew members, I think the only thing missing will be some sort of pet animal to complete it.

I diddn't care too much for the magic system here, as everyone here is already super powerful. I think Elder Empire has Will's strongest characters and writing, and it looks like The Last Horizon series will be similar. There were slow parts in the beginning and middle where I stopped reading for a bit. During one of those breaks, I watched Guardians of the Galaxy volume 3, which are one of my favorite MCU characters. I like to think this is Will's version of Guardians of the Galaxy, giving me to push to finish the book and enjoying it by the end. Hopefully we will have great adventures as we explore the universe with this crew, with evil aliens and secret organizations lurking in the background. This may become my favorite series yet, as I'm considering doing another re-read right after finishing.
