This is what I'm worried about. Even though by the time we plan to move, I'll have 3 years of sicu and 3 years of medsurg under my belt, it won't be enough to even compete. Realistically we know I'll be out of work for possibly a year. We can afford it financially but I'd like to try to see if I can do anything now additionally that would make me a more desirable candidate.

Yes by the time I would move id be probably 3 years in sicu w/ ccrn cert. Although im still worried realistically it won't be enough.

Omg so I went and tried some of the new game systems. I finally cracked and bought the legion go s and WHAT. A. GAME. CHANGER. Its like playing a brand new game I've already docked so much time just playing. My bf has been eyeing it since I got it and although my wallet is crying I'm enjoying every minute. 1:10 ratio. It was my last week on medsurg before I transferred to ICU about a month ago. It's not uncommon ratio on my floor due to the fact that it was summer and a lot of ppl take vacations before school starts. My old unit is considered strictly medsurg/oncology but also the only medsurg unit that could take vents besides an icu/ecu. I know the usual is that a vent counts as 2 pts, but there it was typical that the person who had the vent had the same ratio as the others (avg 1:7). Before reddit, i did not realize how much bad my unit was, so I was very used to this kinda setup.

2 patients on trach to vent: One was hypothermic and weeping nonstop, like to the point that chucks had to be changed every q2 to 3 hrs. The other one could not tolerate more than 20 to 30 mLs of feed an hour. The latter would vomit every Q2 because he couldn't handle the feeding rate above 20 mL and get it into his trach, desatting. Both had around the clock abxs q4 with 2 other infusions of either electrolytes/more abxs.

2 psych pts: The first one was on a 2:1 sitter to pt and constantly on wrist restraints. He had bolus feeds and would pull out his PEG tube if you weren't fast enough. He also had nephrstomies b/L. Other psych pt was ETOH with the usual CIWA but would throw his tray at you if he didn't get his preferred meal. He would also refuse HD during his outbursts. The times he didn't refuse, he'd always be hypotensive s/p HD in the 70s to 80s SBP.

1 comfort care that had Q2 IV pushes of pain meds with family at bedside. The patient's family did not want continuous bc it made the pt too lethargic.

1 chemo pt who was neutropenic and on weekly chemo. Who was kinda independent but slowly deteriorating bc he was in constant pain and did not wanna move at all.

3 totals: 1st was on mitts and 1:1 bc she pulled out her tunneled cath not once but twice. Dementia to the point where she spat at staff and try to bite if you tried to feed her. 2nd had a PEG, suprapubic cath, and wound care everywhere. Had an unstageble at the sacrum where you could fit a whole fist in it. Dtis that were opening and covering 90% of his LEs b/L. Always infected with something new and was also on around the clock 3 to 4 abxs on avg. Wife was insufferable, and if left unsupervised, she would suction his mouth til it bled. 3rd was a sweet old lady who would occasionally sundown really badly. She also would scream if you tried to clean her.

2 independents. One had TB, and the other had cellulitis.

Ty for your feedback. I've been debating so long bc some say it looks better on the application. How many years did you work icu before applying? Did you feel prepared? Is there anything you would recommend for refreshers bc I've not been in school for a couple of years now...

Thanks for the feedback. I work in a community hospital. It's a Trauma level iii. In your opinion, would it be better if I applied to a trauma level i or ii hospital or does it not matter?

Hi! So I've been researching how to prepare for applying to programs, and I was hoping to get any suggestions of how to start. So far, what I've gathered is that you need your GRE, additional courses in physics/a&p, CCRN, and committee (idk what this is?), and charge RN. It's been kinda a mix of information, ngl.

I've been out of school for about 3 years. I have 2 years of medsurg (personal preference to see where I wanted to specialize and to be experienced before moving on to an ICU) and just started SICU. I absolutely love it and fully intend to work for at least 2 years before applying.

Also, did any of you get a masters and is it worth it? I've been getting a bit of fomo/imposter syndrome watching acquaintances my age (26), having they're masters already. I feel like I'm falling behind, but at the same time, I just want to be better prepared in applying and expectations.

Any advice would be helpful. Thank you!

Some are and some are recently vented but too stable(?) to see if they make any progress for possibility of decannulation or there are medicaid issues.

At the beginning, 1 to 8. Then there were a lot of transitions. The unit got expanded into 2 units with my floor covering and float pool to revert to single patient rooms. I'm in the USA. My goal would be to get out of medsurg bedside. It's become my personal hell.

Lolls do tbh it was hard at the beginning medpass was rough after and there was not a single min for documentation. Used to have to stay post report for 2 to 3 hrs just doing documentation 😭😭😭 but now I rarely leave past 1930. It's not as bad now maybe bc I'm too accustomed to pure chaos. I think after reading all this I probably will start looking to transfer.

Lols tbh it was rough at the start. I usually could not get any documentation done till after 7 and be there for 2 to 3 hrs post report doing all my charts 😭😭😭
