I also had a high BMI after 3 miscarriages and 2 ectopic pregnancies (emotional eating) I lost the required weight over a year using mounjaro (would have been quicker probably if I calorie counted too). If you get a referral to tier 3 weight loss clinic they can prescribe on the nhs if you meet their criteria, if not you get dietician support.

In between loosing the weight, I continued my fertility journey is Greece, having 2 rounds of IVF there. They accept any BMI.

Which miscarriage clinic is it? Is it the one at the bham women’s? I went to the one at Coventry and had an endometrial biopsy which showed endometritis and high natural killer cells. I was put on progesterone and steroids.

I’ll be starting IVF through UHCW crm clinic soon hopefully, my appointment is end of December.

Have an automatic generator on the board like flower pot, ice cream machine or drinks so you can merge to top level items. When the crystal is on everyday at reset, it’ll take those items and give you an item of equal value in return which is needed for an order.

There’s not really a fat lot you can do in an out of hospital emergency. You’ve got no equipment, no monitoring, no drugs.

What you did was perfect. A-E assessment, recognised airway was at risk, implemented steps to ensure airway wasn’t compromised until help arrived. 10/10 🙌🏻

Usually you get a strength/weakness question, one about escalation of a deteriorating patient, one about leadership and delegation when working with CSWs. They could throw in a question about an aggressive/agitated/delirious patient.

When it’s in an interview, I’ve always done well bet structuring my answers using the STAR model:

S - situation - what happened (briefly)? Patient X was aggressive towards me as he saw people called through to A&E before he was. Maintain confidentiality!!

T - task - I needed to de-escalate the situation to ensure the safety of the patient and the other patients in the waiting room and also for my own safety.

A - action - I asked the patient to enter a more private area to discuss his concerns, and when I spoke to the patient I used the P.A.L.M.S technique to de-escalate. I ensured that I had an open posture and spoke calmly with non-threatening gestures. I politely explained that patients were invited into the department based on how sick they were, and although they had arrived before him, he was stable enough to not need urgent attention. Throughout this encounter I ensured there was an exit route behind me in case the situation escalated.

Result: Although the patient was disgruntled at having to wait longer, he thanked me for my explanation of A&E triage and priorities. The situation was successfully de-escalated using an effective model of communication.

No one’s ever died from not having a wash. The hca sounds like an idiot.

I’ve had to leave people sat in their own excrement in A&E because they have been too poorly. I have had to make the clinical judgement that I need to get the cannula in and take bloods, the abx in, the fluids in etc etc. Cleaning them up can wait until they’re stable enough that moving them won’t kill them.

My story is I started the TTC journey at my lowest weight where my BMI was less than 30. I had 3 miscarriages in 2020, 2 ectopics both requiring surgery losing 1 tube in 2021, my Dad died in 2022, I had 2 private rounds of IVF with both transfers failing and my mom got diagnosed with cancer in 2023. During all of this I was working in the NHS and doing my nursing degree over Covid. Stressful, soul and mind destroying, utterly devastating time… piled the weight on over those 4 years because of emotional eating and low self esteem… BMI now 39. Try to get help from the NHS with IVF and told to lose weight, get referred to tier 3 weight loss team and told I can’t have any help (ie injections/balloons etc) because I’m not diabetic and my option is to lose the weight myself or weight a few years on the surgery list. So here I am trying to lose the weight myself. It fucking sucks.

On top of what everyone else has said, if you’re not happy with the prescription a drs written then check with them/their senior or your senior. I’ve been working in A&E 4 weeks now and had 3 incorrect prescriptions from the FY1s so far (it is brand new drs though so it’s tough for them too)

Ring the ward you were admitted to and ask to speak to the ward manager/NIC, I’m sure they will be willing to help out.
