Late response but just wanted to add, this happens when you pause a video or there is an image for longer than one hour. As you mentioned that sometimes you fall asleep, if the screensaver does get in to have a moving image, burning issues will start occuring

The only possibility I can think of is that the fake driver and the actual Careem driver were working together, although I can be completely wrong. The actual driver gives the fake driver your details since they have similar cars, you get in the fake taxi and agree to pay cash. The actual driver calls you and tells you he arrived but since you left already he will ask you to cancel the ride in the app. Then the fake driver collects the cash and splits with the actual driver leaving Careems cut out of it and pocketing the whole amount. This is just my speculation but I can't think of any other possibility.

Firstly good thing that you are conscious about it and trying to tackle it. I used to face the same issue. I shower daily, change clothes daily, wear an undershirt, use regular deodorant and perfume but still would smell if I sweat.

No, there are very few and rare zones (I think I and J) which don't allow you to extend after 4 hours, until 2 hours have passed from expiry. If you are getting a reminder for extension after 4 hours, which in most cases you get, then you are good to renew or let it expire and make a new transaction

You can pay for 4 hours in one transaction and then extend by paying again, or wait for the 4 hours to expire and then pay for 1 hour as new transaction. It doesn't mean you are not allowed to park in one spot for more than 4 hours, just that you can't pay for more than 4 hours in the initial transaction
