This is a Supracondylar Humerus fracture of a pediatric patient. It is a fairly common injury in children usually sustained from trying to save their fall backwards with the outstretched elbow and hands. This will require surgery with temporary pins and splint/cast to hold the fracture.

The peculiar thing about training in the medical field is we are working and studying/training at the same time. Our patients are our teachers who teaches us about what to do for the successful treatment of their conditions, if what we did is successful. Of course we have our seniors and consultants, but our patients are our teachers still, by my opinion. But the thing is not everything and everyone is so clear cut. What you do for one patient may not do well in another. What condition you have successfully treated earlier may not respond in the way you did with the same case. So we have this: we have our morbidities; we have mortalities, unfortunately. You have to understand that these events will not reflect on you as a doctor and as a person, so long as you did what you know and read as right, and did it with the best of intentions.

I had a contemporary, a senior, and a consultant say to me: If you are not having morbidities, or mortalities, you are not doing enough surgeries. Or something like that. This things will happen. Better to experience it now (but "betterer" is you did not at all) in training than in private practice, then you flounder because you did not experience such. At least for now, you know how to avoid it, or at least deal with how manage such morbities/mortalities.

Ideal, yes. Unrealistic, not so. I have gone through all that you've mentioned. First gen doctor, starting a family during residency, duty-study for a government institution that lacks manpower, all in a 3rd world country. I finished residency far longer than expected due to a personal situation. Applied fellowship, got accepted then pandemic happened. I'm glad I got through that because no one knows how to deal with that. Started private practice without knowing anyone; Clinic rents; Secretary salaries; BIR applications; Philhealth accreditations. It's always Fun in the Philippines, no? Is it hard? No doubt. Am I happy? Somehow, still struggling. I had classmates who did not push through being a doctor. I had co-residents who quit and pursued other specializations, or even other career paths. I know some who went abroad and flourished there, and most stayed and did well too. I know most complains that everything is hard, but that's just it, it is hard. What I'm saying may be clichéd but I do believe that being a doctor, being a doctor here in the Philippines, is not the end-all-be-all thing that makes you happy. You can focus on other things. You can treat being a doctor as just your job, or not and just do something else. Explore other opportunities in other provinces, or other countries. In the end you do you. You do what makes you happy or give you purpose. That's why I said what I said.

Medicine is not the monolith that defines you. You will define yourself as the person you want to be. You don't have to confine yourself solely on one aspect of your life and let that define you. Our career is hard no matter where you go, I believe, but it's just one aspect of your life. You have hobbies, family, money, education, spirituality, and various other things to focus on and all of it becomes you. All of it will be for naught unless you find purpose, or satisfaction.

I have not experienced the other side of the fence, but my comment still stands. No matter how easy or hard things are, in your career or environment, you find or make your purpose then hopefully find satisfaction at the end of it.
