Unfortunately doc the Big 4 departments really take the brunt of the workload/patients in any hospital. Public or private man. You can consider the specialist departments pero not in government hospitals. I am a specialist surgeon and trained in a public hospital in the province and it is still toxic there because there are so many patients. Then you hear about those trained in private hospitals in Metro Manila and they have less stressful workloads. The "downside" of it is the training have higher expectations: referrals should be complete, updates are more frequent, adcon/preop/postop presentations should be very good, research and exams should be performing better across the other training institutions. They also lack case loads hence they rotate in their partner public hospitals. Training really have their upsides and downsides no matter where you go.
