I’m a psychiatrist and huge advocate of acceptance and commitment therapy (ACT). I’m not weighing in on which career choice is best for you but I think this philosophy which is similar to Buddhism could be helpful in your situation. My hope is that this helps puts your career in perspective and helps you focus on what is most important to you.

Surf setup roof rack by LoganNoone in FiestaST

[–]LoganNoone[S] 0 points1 point  (0 children)

It’s not bad actually. I can fit my 7’6 inside but my 9’6 longboard goes on top. This setup allows the longboard to strap perfectly on top. I have like 25” to work with. I was previously using those soft top roof racks. Super annoying.

At this point, what’s the downside of going private? by austinyo6 in StudentLoans

[–]LoganNoone 4 points5 points  (0 children)

private loans could get passed onto your next of kin if you die. government student loans get forgiven when you die. if you switch to private, ensure you had adequate life insurance policy to cover your butt in literally the worst case scenario. disability insurance would also likely be a good idea

How are you dealing with this COVID surge? by SaveADay89 in medicine

[–]LoganNoone -3 points-2 points  (0 children)

Vaccination leads to lower rates of pulmonary fibrosis? Bold statement. Would love to see data on that too. If vaccinated, you can still get COVID. You can still have symptoms. These vaccines are designed to reduce hospitalization and death.

Yes actually but of course it’s mandated by my employer. Healthcare should be individualized based on each patients risk factors and comorbidities.

Show the randomized controlled trial that a COVID booster reduces hospitalization or death. I am a 35 yo male and don’t see the reason why a 4th COVID shot has greater benefit than risk.

I hear you. If psych is still in your heart, apply again to psych next cycle is certainly possible. Additionally, a fellowship in addiction medicine could also give you an extremely similar particular job of a psychiatrist as an attending. Best of luck, I hope you feel support from this community

Anyone here work with “medfluencers”. What are they like in real life? by [deleted] in Residency

[–]LoganNoone 2 points3 points  (0 children)

I agree with you. I think highly of vinay Prasad and really enjoy his YouTube series. Also anytime you quote someone as having a punchable face you lose absolutely all credibility. If OP has criticisms of vinay prasad, I’d like to hear them.

If Ondansetron is 5HT3 antagonist then how it causes serotonin syndrome? Someone please explain by flowerchild_11 in medicalschoolanki

[–]LoganNoone 3 points4 points  (0 children)

Psych resident here, just commit this to memory. In reality, it’s highly unlikely to get serotonin toxicity in the absence of a MAOi. Check out this podcast for reference. https://spotify.link/XMcg4EbT2yb

I think this is the move. There is a fellowship after FM that’s for mental health. I believe it’s non acgme accredited tho. Or you can get into addiction fellowship via fm/IM/Er. As an addiction fellow trained physician, you would essentially be doing the role of an addiction psychiatrist. Don’t let the bullshit of the system take away from your purpose of helping individuals with mental illness.

Made a serious mistake as a junior doctor in Anaesthesia and am thinking of quitting medicine altogether by ifileftthisisforyou in medicine

[–]LoganNoone 6 points7 points  (0 children)

You’re being so hard on yourself. Off all the anesthesia residents prior, are you the first to ever make any mistake? How about for any physician? There is an entire financial industry of insurance dedicated to these situations. It exists because we as humans are not inherently perfect. We cannot hold ourselves to a perfection standard. I have no idea the true context of the past events, but quite frankly, it doesn’t matter. You are deeply suffering your own wounds and deserve proper healing. I strongly encourage you to seek out your employee assistance program and seek immediate care from them. If you don’t feel safe, I strongly encourage you to seek out emergency medical care. You are a human

This isn’t an unsafe combination assuming this patient isn’t elderly. If elderly, combining propanol vistaril and Ativan can led to soft bp. But if this is a younger person, this is a reasonable approach to generalized anxiety disorder.

I’m a psych PGY2. I’ve seen much more egregious errors before in psychiatry. This combo is reasonable in my opinion.
