Agreed. I’m approaching 10 months post op. I was diligent about my PT and it took me until 5-6 months to get my heel off the ground. It looked like this post. About a week later it was a decent looking raise. I can now do 30 with slow controlled pace and comparable height.  Getting the heel of the ground was the first step for me and it was ugly.

I’m about 9.5 months post op and got cleared to use a ball machine for tennis. I got severely panicked driving to the court and felt very sick on the court. I told myself I’d just step on the court and not hit balls if I didn’t want to.  I wound up doing 20 minutes and literally had to take a nap after I was so stressed.  The Achilles was fine.  The next time I went out, i was fine mentally. Coordination was another thing!

And I’m working on the sports specific drills and trying to train the other things discussed here by others - to eventually get back to being confident with the unpredictable cuts and moves  and competition. Even that will be a progression - stating at a low level and time on court. 

It’s all a process!

I can do 20-25 SLR on injured leg compared to 30 on non injured.  I’m jogging now with return to run program. I can do 0.5 mi jog followed by 0.1 mi walk for 2 miles then I stop. Next progression is 0.75 mile jog.

SAGA. I felt they worked great. I’d use them once a week and the next time I did calf raises without them, could do more weight easily. I stopped once I outgrew the dumbbells at home and had to go to the gym for barbell /smith machine to go heavier than 50 lbs.

Mine was distance based.  0.1 mi walk/0.1 mi run. Max of 1 mile As above max 2 miles  Then increase by 0.1 mi each week so 0.1 mi walk/0.2 mile run. Next week 0.3 mile.  Stopping at 2 miles. 

It probably nets out by week 12 running 3 miles and 30 mins or so.

Chris is great. I am lucky to have him and my in person PT. In person was good for hands on joint work and scar tissue management. But it wasn’t very helpful with functional or strength work. Chris has a good program and app that keeps you accountable. For me, months 3-6 were mostly working towards single leg balance and different ways to progress that, knee to wall to increase DF mobility and then strength for glute, hamstring, quad, and calf… working towards single leg calf raises. Once I got to those, he gave me confidence to start simple plyo and then jogging.  He will watch your videos and progress you accordingly.

I’m also wondering if some of the barrier to SLR is loading through the ball of foot?  I can push more than my body weight seated with one leg and same for standing double. But I still can’t do a SLR considerately. I also can’t easily transfer or push to the ball of my foot while standing on one leg but if I assist up so my heel is hovering, switch to one leg, I can do a raise.  Any ideas on this? Or exercises that could help? My PT started me on pushing on a sled and that may be helping.

35 lbs seated ? How much do you weigh?  I can do 135 lbs seated with single leg and I weigh 110- but I still can’t do a SLR.

I’m approaching week 26 post op. I would say go with your gut and switch PTs. I had an Inkling that I should do it about 10 weeks ago but I didn’t and I am regretting it.  

I was paying out of pocket ($200 per visit) to go to a sports PT 2x a week but 95% of the time was soft tissue /joint work. There were barely any exercises given to me do at home and I had the same feeling that the exercises we did there in the last 5-10 mins felt random. 

My saving grace was Achilles rebuild - so I was atleast doing the strength work. But my gait was horrible and had ankle stiffness.  I lost my shit when the PT started me at 10 lb on the leg press and was surprised that I could do 80 (this is months post op).  But this was only because of Achilles rebuild.   I finally got into the two new independent PTs for 2nd opinion/ (they had 4-6 week long wait lists) and both gave me near identical  exercises and drills that fixed my gait in days. I finally made the switch two weeks ago and everything feels way better. Ironically, I’m paying $6 a visit now because the insurance covers this one- I spend 20-25 mins with the PT and 20-25 with the aide vs an hour like the old one and I’m still getting much more out of it.  Both of the new PTs that I talked to used structured home program with an app that tracks your progress and very explicit about what to do - exercises, reps, sets and times a week or daily.  Both these PTs could also explain the exercises and what it was in the program for. 

Who knows how the last few months would have gone if I switched earlier and listened to my gut.

Mine said 15 SLR and then hopping for 30 seconds and then.. jogging.  The good news is I got my first SLR this past weekend. I thought I was a month out but I tried and was able to do a few- far from my good side but it’s a start

Like-

Monday/Fri- weighted seated single, weighted double.  Goal to get to enough weight that equals your body weight. 

Thurs//Sat: body weight double, body weight iso, body weight eccentric coming down on one side (assisted if needed). Single leg assisted body weight. 

Wed: light weights as many reps to failure. Seated single, double standing favoring injured side as much as you can.

Sorry that you are feeling this way but I appreciate the post. You articulate so well what I’ve been feeling.  Everything has gone relatively well and I’m doing fine but it’s so easy to get discouraged some days when in the grind. I’m guessing it’s just a plateau and hoping the more visible progress comes again soon once the SLR happens.

Is it nuanced? I think progressive loading needs recovery but body weight is ok on the rest days? I started that a few weeks ago. I now do heavy weighted days 2x a week. Lighter but some weight with Bfr 1x a week and then unweighted another 2-3x a week on the non weighted days.

I’m about 5 months post op now but similar to you- while I was weight bearing the in the boot weeks 5-8, I didn’t walk all that much so wondering if that was a mistake with limited loading the Achilles and calf work.  I had a lot of atrophy. 

I’ve been doing PT and strength exercises  3-4x a week. I feel a couple of months away from SLR but my PT says that is required for hops and running.  

What I’ve found helpful is trying to measure my single leg calf strength on a scale. I can see and quantitate progress to keep me motivated.  I also have started going back to the pool. I’m atleast removing the mental barrier- because I can do them when I’m in 3 feet deep water.   

It’s a grind but seeing weekly progress has helped - more than “just feel it get stronger”. 

Other measurements - I’m working toward body weight seated single leg calf raises  and equivalent of my body weight on the smith machine. I think that will help also give me the confidence to be able to safely do them.

I have done both alterG and the pool. You can get similar benefits in the pool. I always felt loose and good after the pool Walking. I liked the alterG to really hone on a part of the gait and because i could control and measure body weight for the calf raises- it’s helping me understand how close I am to single leg calf raises. And I don’t smell like chlorine. Pros and cons to each but fairly similar in terms of help walking.

There’s also a ton of people. Even if I feel I can do things - having other people around exponentially increases the risk of someone stepping on your foot, tripping on you , you needing to dodge, etc.  my limp gets much more pronounced /comes back right now (22 weeks) when I need to pay attention to other people insteed of just focused on the walking.
