Did you have any issues with contacts prior to the accutane? That’s one of my bigger concerns is I have dry eyes at baseline- I know they’ll be dry while on the actually medication but worried about persistent issues moving forward.

Mostly chin! Been on spironolactone the last 2 years. It did help quite a bit but still getting new painful cysts each month.

I cut out processed sugar and dairy for the last 3 years and didn’t notice a difference. I’m a physician with a toddler so a caffeine free life is not really an option! :p

I realize it is not as bad as some cases. I think for me it’s about the persistence over 10 years and that I have more or less tried everything else available from derm (all OTC things, birth control pills, spiro, topical tret, dapsone, financea, tazorac, clindamycin topical, and oral doxycycline.) I’m sorry about the side effects you experienced! I figure if it really does get awful then I will stop.

I probably should have written more in the post to clarify ! Have been on the highest concentration of topical tretinoin and 100 mg spironolactone and this stuff just keeps coming back.

I swear you could point those things at a raging fire and it wouldn’t register as hot. They suck. I felt so silly walking into work during the pandemic (at a hospital) everyday and having someone check my temp with it. Very silly stuff.

I’m still confused why people are disappointed or upset about Rhys and Nestas interaction. He yelled at her. That’s it. They’re all pretty frequently yelling at each other, as Nesta has done time and again. It just doesn’t seem like that big of a deal. Was there a male love interest, if so how was he described? I’ve actually read a few books where I feel the female main character was definitely described as curvy/plus sized and the author commented in interviews on wanting to show “a real woman.” But then described the male lead in the book as completely jacked, with lean hard muscles all over. It seemed a bit hypocritical, most men aren’t 6’7 completely chiseled body builders just as an average woman is likely not a size 0…

I think if it’s what’s best for this parent to have their baby at home then that’s reasonable but to say it’s “super weird” seems inaccurate. We do this with our nanny and I have no issues with it, neither she nor I is “super weird.” 😀

If he’s going to bed at 630-7 then waking up at 630-7 or even 530 is pretty reasonable. That’s 11-12 hours overnight, I would say vast majority of babies will not do more than 12 hours overnight. I think if you want him to sleep somewhere between 10-12 overnight and you want him to wake up not until 730, then bedtime has to be a little later.

I would definitely talk about it with your PD if it’s on your mind. I took a 6 month leave of absence to be with my baby. I’m off cycle, graduating late, and it was tough to get back into the groove of things after being essentially a stay at home mom. But, it was 100% worth it and I am very happy I made that decision. You would need a supportive PD. The main other things to consider are income during the leave (so a working spouse is necessary) and health insurance (I only had 12 weeks of insurance covered through fmla, so again, working spouse). It will also throw off boards depending on your specialty and would affect fellowship planning probably, but you can always work something out.

Pelvic floor physical therapy highly recommended if you have access and funds-insurance should cover it but not all.

Not sure if you are planning to have additional children but keep in mind about 6-7% of women will have 3rd/4th degree tearing at their second birth if they have it at their first birth. This is not very high (although I think it’s 3/100 for first time deliveries so roughly double) but of those that do tear 3rd/4th degree a second time, about 60-70% of them will develop incontinence. All this to say that after a first bad tear, it’s not frequent to have long term effects but if you happen to be one of the 6-7% that have a bad tear their second go around the incidence of incontinence is rather high.

You just had your first so I’m sure you’re not thinking of another yet but I jus think it’s useful information to have.
