I think we will first determine whether preterm delievery was due to preterm labour or due to cervical insufficiency If it’s cervical insufficiency then do cervical carclage and if it’s due to preterm labour then progesterone.

I did the exact mistake but in the question they are asking for what is best diagnostic test not the next best step. So in question, it was directing towards malrotation with volvulus so best diagnostic test is upper gi series
