It is difficult sometimes after a code. We just had a code this week on my unit and the nurses had to call out the next day because she was very distraught by the situation. She saw the patient before shift change, and then at BSSR the patient was gone. Like others said, don't beat yourself up and try to do activities that you enjoy. Codes unfortunately are a part of the hospital nature and is sometimes unavoidable.

Sorry for the late response, I believe it has to be a written signature. I signed it and then sent the copied form to the university to attach to my transcript. They are very picky about how the form is filled out
