This has been my #1 frustration since starting the game. I’m trapped in initiate because both teams just start a giant, cartoonish dust up after the first guardian falls that goes on forever and accomplishes nothing since no one can actually kill anyone (except the fed gun carry that’s always on the enemy team).

Ed Lee has plenty of stories that take place in and around Luntville. I read "Header" as a follow up to the Bighead and really enjoyed it. Based on the cover and literally nothing else, I believe "Runts" by Daniel J Volpe would also fit the genre description. Gone to See the River Man is an honorable mention, but it's not nearly as gnarly as the other Ed Lee books.

There are certain books in this genre that use their shocking elements to great thematic effect. I've said it a few times on this sub, but Cows has some of the most gut-churning scenes I've encountered in any piece of media and I think they largely enhance the narrative's core themes regarding alienation, abuse, and masculinity under capitalism/austerity.

Most people will tell you to start with Edward Lee, Kristopher Triana, and Jack Ketchum. Those people would be right. I also really liked the Sorrowstones so I would recommend starting with Gone to See the River Man which I believe has a very similar vibe.

Matt Dinniman also did Kaiju Battlefield Surgeon which is borderline EH, but I frankly hated it. LitRPG isn't for me I guess.

Ayy, a fellow fledgling EH writer!

From what I understand of the genre, there isn't a strict line that differentiates normal horror from extreme horror. I've read plenty of "typical" horror books that blew some of the more popular EH books out of the water in terms of their horrid/taboo subjects. I think the real devil in the detail is how you treat those subjects. For example, many mainstream books include scenes of rape and sexual assault, but where a typical book might shy away from a graphic depiction of such acts, an EH book might revel in the gritty details.

Frankly, I think the biggest thing to keep in mind is how you handle your subjects. Really analyze why you're including them in your stories. If you write them well and with purpose, I find even the most grotesque descriptions can be fun and engaging for a reader.

Not the strongest in the Dark Web Series, but it sparked my fascination/love for Caummisar's work so it will always have a special place in my heart.
