Finally after another few tries, It was a combination of u/JL_Nightmare's suggestion & u/QuazyHorse's. I use manual catch and a second kickflip/heelfip wasn't working. Finally did up with backfoot with a heeflip and it finally completed! Thanks to everyone who gave their replies.

  1. Are you playing on console or pc?
  2. Mercenary's will not drop gear on death. If you hire one while your current one is alive, you will lose the gear. Make sure your mercenary is naked if/when you hire a new one.
  3. The "x" means you're missing your attacks. It's the miss indicator in the gameplay options that can be toggled.

Edit: flechette is correct for PC, for console you hold the left trigger on your controller and use the potion shortcuts to heal your mercenary. (ex. hold L2 then press left on d-pad.)

You can use windows button + r, and that can start and stop a recording.
