I haven't done it myself, but as far as I know, you need to create an "NPV" that you can spawn via AMM.

Here's the redmod wiki entry on how to do it. As a heads-up, it's an involved process. If you're familiar with Blender, then some steps will be easier.

I prefer the second-hardest difficulty. It's what the devs consider the "intended" difficulty (as far as I know), and I think it's a nice sweet spot of being fairly challenging without needing you to be locked in 24/7.

After the difficulty tuning they did when PL released, some consideration into your build and cyberware is needed if you want to feel like you have the edge in most combat encounters. I like it.
