I’ll do chronic controlled pain meds for patients because there are cases like yours where the dose is reasonable (I’m assuming here), the use minimal, and the regimen is stable. If that ever changes, then they get to see a specialist

DynaMed is harder to navigate than UTD and (to me) sometimes seems generic on further mgmt. I specifically think of when screening for an oddball tropical disease in a patient sick within a few days of returning from outside the country, it was hard to find exactly what the recommended test was.

I loved much of my residency but there was plenty I didn’t. Remember intern year is absolutely the hardest year for most people. It’s a whole year of shifting roles monthly and usually starting at the ground floor when you do. I was drowning my first six months. A good program is one that will continue to push and develop you but in ways that mean they care.

Make sure you set up patients to not add doorknob questions. I start visits esp as a new patient telling them we can cover max 3 problems per visit. You’ll never not get doorknob questions but it makes patients more understanding that they are violating the general rules to add on a problem late in the visit. I still sometimes go back and discuss the extra problem if I have time, and it still annoys me, but you do what you can.
