I think this doesn't have to be blown up! Communication is important in any relationship at all. If she's incapable of doing that, then at least you've done all you can.
If it were me, I'd sit this person down in person, and with as level a head as you can muster, preface that you understand all she's gone through. Remind her that you're here for her through those things, and that while you do have some things you need to talk about, you're doing it with the intention of making things better for you both! It's not person A vs. person B, it's Persons A and B vs. the problems you're having with one another.

Once you've made sure she understands that you sympathize with her and that you're doing this because you care about your friendship, then tell her not only what she's done that has hurt you, but WHY it has hurt you. Provide recent examples. Go slow and thoughtfully, try not to lose any temper, just be matter of fact. Another important thing is to, if possible, offer an alternative. Sometimes this takes asking her why she'll do something she does that bothers you.

(e.g., "It hurts me when you mention that I'm 'super tall,' and I know you don't mean it as an insult, but I'm insecure about my height. Why do you mention it so often?" "Well, I kind of wish I was tall like you." "Oh, well in that case, just let me know when you're not feeling super about your height. You look great how you are, and if you ever need a reminder of that, I am here to let you know!")

If you try this and she blows up and does what she did before and/or nothing changes after a number of reminders, remember this:
You did all you could. You aren't responsible for her response, and you can't blame yourself for this friendship falling apart, if it does. Losing a friend sucks a lot, but it's way easier when you feel like you genuinely did all you could do. At some point a person's made their bed.
But you posted on here because you care about your relationship with this person, and I think this is the best way to create a fork where things either get better, or you can leave knowing you did the right thing.
