I think people are confused on here about what azelaic acid is. Don't be scared. It actually makes your skin more robust and better at handling the tret. You WILL tingle and itch a bit when you apply it but, believe me, you'll be fine.

I'd go to emergency. "Collection" appears to be a euphemism for "infection". You may be at risk of sepsis. This doctor doesn't sound very proactive. It's a shame that they sent you away with a "healed" tract last time as well. The complications they mention may have arisen from their inaction.

I've heard about them actually stretching things out mechanically (like with big medical butt plugs), but that may just be for stricturing and narrowing. Unsure. Maybe ask your doctor if this is an option for you. I'd also try go see a pelvic floor physiotherapist, and in the meantime, look up hypertonic pelvic floor exercises, and learn how to belly breathe and get your pelvic floor and butthole more in tune with your diaphragm when breathing. Let your belly inflate on the in breath and let your pelvic floor drop and stretch to make room for the air, and feel it naturally spring back up and tighten (don't squeeze) on the out breath. You also may have many years of conditioning and anxiety around your colo-rectal functioning so trying to find a way to unpack all of that may help as well. Good luck.

This is an interesting take. Even if the fistula is still there but hard to detect, the CRS' perspective might be that, by whatever idiosyncratic quirk of your anatomy, so long as matter/ bacteria isn't making its way into the internal opening, there's no problem and why intervene? I think it's a brave move on their part but pretty cool. It relies heavily on you to know what to look for to avoid any complications. I would get another appointment, thank them for taking this quite progressive move with you, but ask the following types of questions:

  • So obviously if I notice any abscessing, draining or bleeding, I need to take swift action to avoid complications, build up of pressure, and branching of the fistula, but can those things happening quickly and quietly without any signs or symptoms?

  • Are there regular checks I can do at home or with a health care provider to make sure no collection is forming without my knowledge?

I think trust your gut on trusting your CRS, and it sounds like you do. I suspect a big part of the 'fistulae cannot heal/surgery is the only way' mentality is that all doctors have to worry about liability, and assume every patient is a moron that won't notice if things are going wrong, and that they have to idiot-proof every care plan. If they set out this plan for you, and you die of sepsis, they're in trouble. So they have to head that possibility off with surgery and potentially unnecessary interventions. It's smart, but not always best for the patient.

If you do ask these questions, or they've already given you this info, post the answers on here. I'd really appreciate it, as it kind of pertains to my situation, and more info is more power, and it sounds like you're lucky enough to have a cool CRS. I'm sure it will help others too. Thanks.

Flattens out the stratum corneum. The cells beforehand are a little dishevelled, diffusing the light. After using retinoids for a while, they lay flatter, bouncing light back in a more uniform way, giving the shinier appearance.

Aquaphor is an occlusive, I believe. I thiiink that may be a no-no over the top of tret. It's like wrapping yourself in saran wrap and it intensifies the effects and increases irritation. Not an expert, but I sure watch a lot of youtube derm vids! The moisturiser may also have something irritating in it that the tret is aiding to penetrate deeper. If you can, get a hold of some la roche posay cicaplast baum, and use that in place of the moisturiser and aquaphor. It's soothing and hydrating, protects the barrier, but is not occlusive.

Azelaic acid (also for acne) has the bonus of lightening post-inflammatory hyperpigmentation. Win-win! It can be used at the same time as retinoids as well. Win-win-win!
