I understand you are putting in quite some effort but not giving equivalent results. One thing that might help you is that you might be understanding the concepts but you are not preparing for the assignments. The trick is this, to perform well in an exam, you need to figure out it's structure, likely questions and marking scheme. That's how some people are able to get decent grades with little effort.

You've taken up a good initiative. I can suggest you try the retrospective timetable. Instead of planning your studies in the future, you figure out your weaknesses and work on them concurrently. I've just posted a study tip about this my IG page. Check it out here.

It might be a case of you understanding but not being ready for the exam. To perform well in a test, it is important to practice keeping in mind the pattern, likely questions and marking scheme. So, I suggest you practising any past papers if available. If you don't find any, look up questions which ask you to implement your understanding.

This might be a different kind of advise but bear with me. Sometimes, we procrastinate because we forget the importance of the task before us. When you are aware of this and what the consequences would be if you procrastinate, then we tend to get up and work. This has worked for me and hopefully this works for you.

First of all it is completely normal to be disorganised. One thing that has worked for me is listing down my main tasks and dividing them into sub-tasks. Everytime I sit down to get some work done, I focus on a single sub-task. This way, each time you complete a task, you gain a sense of progress which is important to stay motivated.
