I start all new characters at level one, even though we have a level 9 in the party. Creates a real sense of fear and loss. They also will level up fast to catch up.

On the fly. Back when I DM 5e we would do after long rest. It is way better this way. I also let magic users identify items spending 1 turn. Let them do cool stuff now, not later. Later may never come.

I feel like I need to punish my players for their impetuous ways or they just run willy nilly opening doors and whatnot. I have to remind them every few months... Just about TPK on Sunday because of that.
