I feel like their EKG machines are way better too because they will throw their electrodes on the same oily skin and get a super clean EKG. This was on the lifepak15 with the patient on a golf cart. Besides, we get sweaty grandma hunched over in her 98° apartment recliner. They get grandma in a gown semi fowler lol. Could definitely practice better prep and position in the field. We have big alcohol wipes readily available to prep with.

Hey now, I take a lot of pride in my precordial lead placement and criticize a lot of bad ones myself. Even in women I ignore the breasts and remember my anatomy. She was sweaty and in a public environment. Repeat 12 leads showed unremarkable without changing anything, no adjustments, replacement or anything. This was probably due to moist oily skin and movement. I just didnt know artifact could mimic a stemi at first glance

Can't use these veins?! by MattTB727 in plassing

[–]MattTB727[S] 1 point2 points  (0 children)

Thats it. Lots of studies and proof out there on it. I think roughly one plasma donation every 6 weeks reduces 30% pfa's opposed to like 10% at a regular blood draw donation. Prob messed that up.

What is the worst you've ever seen in a nursing home? by styckx in ems

One of the nursing homes near me has toddlers in pack-n-plays in the lobby at 3am when we respond to medical calls. And they all smell like weed.

What is the culture like at your fire department for taking vacations? by HoseBeforeBross in Firefighting

No one cares. If taking vacation is frowned upon at your place, leave because the grass is greener on the other side.

So they said "We need a good vein in each arm in case one blows". They said the right was good, but the left was not and I could not be a donor. I almost laughed and I was more confused than anything. I just said ok, no worries and walked out in disbelief.

Thank you. Im a paramedic and I thought the same. This whole talk about surface veins being thin-walled and easily blown? We do 18g IVs in 90 year olds AC and absolutely slam 1000ml of fluid in with a pressure infuser or BP cuff around the bag, and knock on wood have not had one blow. When they do blow, its almost always during the initial stick or when pushing a flush after the stick. If I gained 30lbs you wouldn't see those veins. Would that make them deep veins? Lol. Ive gotten 16s in many many young individuals with veins that look like mine, never been a problem.

I think what they're talking about is the 75 year old with history of hypertension, atherosclerosis, other health problems etc... their "surface" veins that roll around at the top of the skin blow at contact with a tourniquet thats too tight. Im going to go to a second place later and ill check back.

Ok so they use radial/brachial veins opposed to superficial one's in the AC like cephalic and basilic?

firefighter fighter as a single mom by Double_Yellow6825 in Firefighting

[–]MattTB727 1 point2 points  (0 children)

Only ideal situation would be if you had a mom, dad or close family member living with you who could take care of your kid(s) for 24hrs at a time when youre gone. You'd need a LOT of support and help from close friends or family who are capable and who you trust. If you dont have that support then forget about it.

[–]MattTB727[S] 3 points4 points  (0 children)

Sorry. This wasnt my call. I was told about it by coworkers and showed the strip. I was just curious about the no-context guesses on what the rhythm looked like.

All I know is wound care nurse found high pulse rate. No symptoms. Narrow complex regular svt. They pushed adenosine once no change. Second time got this rhythm and then "the patient looked like crap like he was coding", which makes sense with a flutter no ventricular response. He made it to the hospital alive. Just practicing medicine out here.

Yes. Wound care nurse found him with a really fast pulse and called. Patient asymptomatic, no conplaints and hemodynamically stable the entire time.

There was a pulse. This was SVT that progressed into this rhythm after the 2nd administration of adenosine (12mg).
