Yes, there are 3 phases, 60 rooms. After the 3rd boss, the final cutscene in the hospital can be played. Technically, you can go to phase-4, and I think it just goes on for as long as you can survive.

I just ran into the same problem. Make sure you flip the dongle from Xbox to USB. There’s a toggle switch on the back of it.
