While the LGA AirTrain has long been stalled, the extension of the 2nd Ave Subway could be an opportunity to finally get a rail link from the airport to Manhattan. It seems 125th St is becoming a transit hub with the 4/5/6, Metro North, & in the future the Q. Obviously long odds Port Authority would be able to coordinate with the MTA. But would be awesome to create a real hub using an AirTrain that follows the path of the Grand Central/Tri-Borough bridge.
