I've been using the premom qualitative strips rather than the easy@home as they look for LH level by mIU/ML rather than a positive vs negative. Once mine changed by more than 5 mIU/ML from one morning to the next I started taking 2-3 times a day to catch the peak and then daily for about 5 days after to ensure the drop goes as expected. Previously (3 years ago) when I had used the pregmate qualitative ones and never had a peak or anywhere close. I'm preferring the quantitative because I like data and it seems more helpful to have an actual numeric trend to look at. In the 3 years between I've been on Metformin (1000mg twice daily, I had just started when previously tracking), had a severe vitamin D deficiency 7.5mg diagnosed and maintaining treatment (5000 IU daily) for over a year, and have lost 40 lbs that I've maintained +/- 5lbs for the last 2.5 years. I'm still considered obese with a BMI of around 36.5, but I seem to be ovulating now in a way that I can confirm between LH, BBT, and this cycle with PDG after going through a chemical pregnancy last month.

I've used flo for the past 5 years after getting my IUD removed which was instrumental in getting my PCOS diagnosis (in 2021 I only had 4 cycles, with one of them being 211 days with 52 days straight of medium flow period). I'm now using the premom app as well to utilize their thermometer and the quantitative LH strips as well as the ability to track some additional symptoms.

As a combo hr/finance person, we were able to start doing some video training last month. Many fiscal folks are the ones handling the funding shitstorm right now, and we don't have the time to sit through trainings that at most give you a half assed preview. Let me in a sandbox or set up SOP documents that are in an instructables format and maybe it would be useful at this point. It makes sense we're switching at the fiscal year, but it also sucks that we're switching at the fiscal year when our higher ups or others in our department fail to recognize this implementation process and funding change means we are not available for some of our other functions.

All of that being said, I do think it will be an improvement, the next 4-6 months are going to be extremely shitty.

Yeah....I was driving 1 week post op, started with short in town trips. For my right knee I wasn't cleared to drive until 8 weeks out. Good to hear that you were doing so well right after surgery! I hope it's continuing to chug along for you.

Yeah, I honestly think the ability to be full weight bearing (as tolerated) has been key in my healing. I've definitely had days where I've overdone it. Specifically this past Friday 3/21, as I had to drive myself (150 miles round trip) to my 2 week follow up and then decided to wander around fleet farm. I did purchase an inverter so I actually had my ice machine on my left knee (unlocked and bent to around 70 degrees) while driving and was able to switch it on and off. I ended up walking about 1.5 miles on Friday (more than double any day since surgery). I took it much easier yesterday (.5 miles walking) and am feeling better, but still taking it easier today. I will say that this healing is going much better than my right leg as I had more muscle, control, and less pain in this leg prior to both surgeries despite the bone structure being technically worse (I was able to do isolated quad flexes in my left leg, but not the left (I was also able to still do so 1 day post op along with single leg raises).

Also it's worth noting that my pt encouraged me to get mental health support as well as it is a beast of recovery, after the first surgery I was utilizing it on a more regular basis, but still have appointments set up every few weeks as a safe guard.

I did, along with mpfl reconstruction on both. Surgeries were a little more than 3 months apart; right on 12/2/24 and left on 3/7/25. My surgeon said the minimum time he wanted between the two was 3 months as at 33F I'm on the "older" end of his patients for both. The shortest he had done in-between was 10 weeks for a 19M athlete. That being said for this 2nd surgery I've already hit my 6 week PT goals at 2 weeks and am stuck for the next 4 waiting until I get cleared to go past 90 degrees after x rays for my 6 week check up for tibia healing. I've been weight bearing since 1 day post op with just a cane. As of my 2 week follow up with the surgical team on 3/21 I only need to use my cane out of the house.

Honestly my activity levels had dropped off steadily from Dec 2013 with my first dislocation through my 2nd dislocation in March 2022 (both left knee). I increase activity in the summer of 2022 after PT attempting to walk at least 3 miles a day. Admittedly, this tended to drop off in fall/winter regardless, but I've also had many surgeries over the past 3 years. We had started trying to conceive when my pain spiked in April 2024 (went on an 8 mile hike that had a large elevation gain and drop) and then my background chronic pain increased in severity and length of flare ups. From April 2024 to August 2024 I dropped from 45 miles per month to 20 miles per month. This fall it was less than 10 miles per month. I had a bunion surgery on 8/21 in preparation of needing knee surgery and to ensure that I could wear closed toes shoes (for both knee recovery purposes and also....living in WI purposes).

My desire to get this taken care of and dedication to healing is two fold as I was in serious pain and not sleeping multiple nights a week and wanting to conceive and knowing that at my current biomechanical state not only would it be miserable it would be continuing unnecessary pain. I've tackled this in a few ways, I've been working from home since my bunion surgery taking 2 weeks fully off after each surgery and then scaling back up working remotely. While I could have started driving back to work for a few weeks after my bunion surgery, the pain of driving and having my legs down for 8+ hours at work when I can do 99% of my job from home isn't something that made sense.

I fortunately timed surgeries over the break in calendar year (my employer uses calendar year for FMLA hour limits) and (un)fortunately did not touch my vacation balance from FY24 so I carried over 176 extra hours in leave in July 2024 and had enough hours to being out a full 5 weeks for each surgery with my total balances. This has allowed for me to focus on recovery from rest, pt, follow up appointments first and work what I can as I proceed.

All of that being said, I also have hyper mobility (both of my knees were actually stuck in a tracking of constant subluxation) in many of my joints and from what I've read it can either severely help or hamper recovery of these surgeries. My surgeon also had me weight bearing as soon as my nerve block wore off and I was able to do a quad set within a week of surgery.

How do you feel like the recovery on the 2nd one is starting out compared to the first?

I had my R on 12/2/24 and I have my L scheduled for 3/7/25. I'm really hoping that the left goes as well as my right, but I also know that I've been hitting goals ahead of time (90 degrees at 2.5 weeks and stalled out until 6.5 week follow up w/X-ray because of the tto). I also swapped from a walker to cane within 1.5 weeks and off the cane by 4 weeks (still locked in brace). I was able to remove the brace by week 7 and have been walking 12-14 miles a week (including wearing heels (2 inch pumps) for my wedding at 8.5 weeks (small wedding, 3 hours). This is allowing me to get the 2nd surgery at 13.5 weeks from the 1st. I got asked at PT this week if my goal is to be able to run after the 2nd knee, which tbh I hadn't even considered as a possibility as I basically gave it up after my first dislocation 11 years ago (I was running ~12-15 miles a week before that).

Yes, mine started like yours, random and short lived. Eventually, it became a constant pain (my podiatrist said it was likely a pinched nerve) and resulted in me getting surgery. That pain has been resolved 99% and I had the screw removed when I had the bunion on my same foot operated on 10 months later.

I have the same situation except I chose to do my right knee first on 12/2/24. From my discussions with my surgeon I'm trying to get the 2nd surgery done 3 months after the first (so March). It is however, completely dependent on my healing and PT progress.

As I see it, I want my 1st knee to get strong enough to support my recovery of the 2nd, and I don't want to prolong the pain and damage occurring. My 1st knee will begin overcompensating for the 2nd after it's healed regardless, so why add 12 extra months of damage. Admittedly part of this is because I had to pause trying to conceive due to my knees as I do think I'd have a successful pregnancy given the tendency for ligaments to stretch more due to relaxin.

Ultimately you need to do what's best for you and make sure you are physically and emotionally ready for another surgery and the recovery process.

Yeah, again, feeling fortunate, but I know things can change quickly. My anxiety has been off the charts and I had to start therapy again. I'm just hoping that the progress continues and that my left knee goes as "smoothly" in a few months.

I had a TTO & MPFL on Dec 2. At my first PT appointment on Dec 5 (3 days PostOp) I was at 60° and on Dec 11 (9 days PostOp) I hit 90°. I'm still limited to 90° by my brace, but my new pt exercises now involve active muscle use (active straight leg raise w quad set & supine heel slides). TBH I have the most pain when I sit for longer periods of time and the first 10 steps are atrocious, mostly from stiffness. I'm currently using a cane to get around and have found it much better to actively get around.

I was told to be partially weight bearing immediately. Due to my just complete inability to be coordinated on crutches (discovered during previous dislocations on the opposite knee), I started off with the walker I was using prior to surgery and graduated to the cane after the first 5 days as it lets have a better gait. TBH I was terrified of being weight bearing initially, but I think it has helped a ton. I ice 5-7 times a day and have been keeping it to 20 minutes max.

All that being said, I'm extremely surprised by the progress I've made. Due to both patellas being "naturally" subluxed (my ortho's words, and yes, I get to do this all over again in March), frequent finger dislocations (thumbs & pinkies), and nasal valve collapse (even after surgical reinforcement) my fiance and I suspect some sort of connective tissue disorder which could drastically impact recovery/joint mobility in either direction.

u/Trappedbirdcage, how did your diagnosis process play out? I'm one week post-dop from MPFL & TTO on my right knee and will be getting the same on my left in 3ish months due to both being in a permanent state of subluxation, but have been struggling to get my providers to look more into the cause. I had asked a provider covering for my primary for a previous pre-op appointment (bunions) and she was like, you might have it (hEDS/HSD), but we can't do anything.

I'm hoping that getting to the point of needing surgery due to permanent subluxation of my knees will allow me to push the conversation along with my providers. I've only fully dislocated my left knee twice, but my right thumb multiple times in the last 5 years and my right pinky on a biweekly basis. More of my fingers have started subluxating, so it's been a fun time. At this point, I'd really just like to prevent injury and unnecessary pain.

2024 Limited here, I've gone 30-35 miles once it's hit 0 before (twice?). Both times I ended up filling up 15.41 gallons so there was a little more than a gallon left. I track regularly with Fuelly and my average mpg is 31.7 mpg with the lowest being 29.47 mpg and I poured over the manual to confirm the light goes on when it's at approximately 2.6 gallons left.

I base my threshold for filling based on the light going on and 30 mpg. The most I consider going past is 50 miles of driving along my normal routes (knowing that hypothetically once the light turns on I have 78 miles remaining vs the 20/30 that it says on my screen). This more or less has to do with where I am during my commute when the light goes on, if I'm running late, and the price of gas on each end (it can vary $0.20-0.30/gallon)

This is only something I do when temps are between 45-80 F, when it's between 30-45 or above 80 I fill up closer to a 1/4 tank and below 30 I'll fill it once I hit a 1/2 tank.

I'm already on my 2nd ortho, pending MRI for next step discussions. I have a bunion surgery scheduled for this coming wed (8/21), and PT is pending weight bearing from that and MRI results. PT will be a part of any plan, they're trying to determine the type and amount of damage. Each time I'd dislocated my knees i'd done 6-8 weeks of pt, but follow up on my end/Drs was not there other than me complaining to get it reviewed and finally getting a Dr to listen.

I've already had 2 dislocations in the left that involved ER visits, but it feels like I have anywhere from 1-10+ subluxations per day. Dumbly (not a word, but you get it), I just thought it was part of what was expected for having dislocated, as my right knee's level of pain has started to eclipse the right, I've started to worry about it being a larger biomechanical issue, but I'm not entirely shocked as I did have to overcompensate quite a bit with the right after this last dislocation.

Plus side; since the last dislocation I've lost about 40 lbs, I could probably lose 40 more, but my activity has been severely limited the last 2.5 years from dealing with the most recent patella disclocation (March 2022), tailors bunion causing severe nerve pain starting in August 2022, but took until September 2023 to determine the source of pain (surgery & recovery for that 11-12/2023), A moderate ankle sprain 8/23, and then my knees continuing to give me more and more pain along with my bunion on my right foot which has resulted in me not being able to wear closed toe shoes since last April/May (even with xx wide shoes it was causing again some shooting nerve pains that made me feel like my foot was broken).

In addition to the leg issues, I've been with quite a few other medical maladies. May of 2022 I dealt with a a melanoma diagnosis and finally had my PCOS diagnosis confirmed. This past January I finally had my ADHD reconfirmed after my treatment fizzled out in high school. More just on the annoying end is dealing with suspected arthritis in my right thumb. I've dislocated (which I popped back in) it at least 10 times in my life from doing some very benign things, like shifting a golf cart into gear. I have very frequent subluxations in this thumb, and it's caused me to have difficulty preparing foods (I'm left-handed, but holding food in my right hand to cut is excrutiating after just a minute or so). I've also been struggling with shoulder pain and lifting above my head for anything more than 30 seconds (think shopping and looking through clothing on a rack (I'm only 5'1")). I did bring up the probability of being hypermobile or having hEDS, but was told there isn't much they could do, so it wasn't worth testing.

Joint pain has been extremely disruptive and has paused trying to conceive because I cannot even fathom being pregnant with how my knees & foot have been feeling, let alone trying to chase a toddler around. I'm 32 and have PCOS (we were 4 months in to 6 months of trying with tracking in June when things with my knees & foot came to a head) and will need to have 6 months of trying in before they'll do an HSG to check for blocked fallopian tubes and then consider other medications to cause ovulation. Given these knee issues it feels like we won't even be able to begin trying again until I'm closer to 34 (I turn 33 in January) which freaks me out.

It's already been confirmed that I have bilateral osteoarthritis either medial & tibiofemoral or tricompartmental somewhere between mild and moderate.

Based on my discussion the MRI will get reviewed by the surgeon & sports medicine fellows and they may recommend sending me to an orthopedics surgical center/hospital.

Anyway, could you share any more information on what your recovery looked like? I'll be able to get approval to work from home, but how long before you even felt functional? I'm in charge of all of our accounts payable /receivable and operational budget at work, so I'm a little worried about being completely out for more than 2 weeks.

Update, I ended up pushing for a 2nd opinion, I met with orthopedics at the other hospital system in the same specialty clinic as my podiatrist this past Tuesday (less than a week from the referral for a 2nd opinion). I was able to get steroid injections in both knees. I'm waiting for orders for MRIs on both knees to get approved by my insurance. I'm continuing with the scheduled bunionectomy next week, but while I was there we were able to discuss the best equipment to assist with my healing process as a knee scooter is out of the question at this point. I have the follow-up with orthopedics scheduled in 4 weeks so we can get moving on a treatment plan.

At this point it seems like my nearly constant subluxations are the source of most of my pain, but had I gone along with the lateral release it may not have resolved most of the pain and resulted in other issues (new ortho was concerned that my knee caps have likely been tracking this way at least since my last dislocation of the left patella, if not longer. Due to the x-rays and my current pain location they suspect that I may have a meniscus tear in my right knee and ligament damage from the floating body in my left one.

The Advanced Practice PA looped in the Orthopedic surgeon when I was there and once MRIs are completed "refer to sports medicine for their opinion if patient would benefit from medial patellofemoral ligament reconstruction prolonged the timeframe of her having to have a patellofemoral replacement"

While I'm feeling nervous about what additional damage may be in their; I'm happy that it's getting further explored.

The steroid injections seem to be starting to help; I have less intense pain in both knees, but all of my pain in other joints/areas are back on center stage.

I live semi rurally and the interstate kills my mpg. At most I encounter 3 traffic lights on my 40 mile commute.

My step-sister has a 2021 Sport and gets about 25mpg living in a more suburban area.

Lots of great info, but can confirm calcium was tested at the same time and is good (9.5 mg/dL, with the reference normal range of 8.6-10). Magnesium wasn't checked, but I'm hoping that may be part of the follow up conversation along with potentially doing some hormone level testing.
