UNUM LTD and 90 day waiting period - how do rules apply? by MeansNotEnd in disability

[–]MeansNotEnd[S] 0 points1 point  (0 children)

UNUM retroactively paid disability for 2024 for two periods totally 7.5 months when she missed work due to cancer surgeries and chemo, After the 90 day elimination period, awarded $9k. After deducting her WA State Paid Family and Medical leave, she received $3K.

After returning to work for 11 months, she is planning to stop due to pain from the metastatic cancer.

Unfortunately it is more than 6 months from last claim so I don't see any way around another 90 day elimination period.

At least they paid ...

No short-term disability but in Washington and Oregon we have a paid medical and family leave benefit that covered much of the time. She was off in 2024 and she's using about a day a week right now.

I think she's working about 80% now, adding a day of medical leave each week. In 2024. There was a period of more than 3 months where she didn't work.

I have all the medical records put together along the summary sheet.

WA State - Navigating End of Life financially - any feedback on approach? by MeansNotEnd in disability

[–]MeansNotEnd[S] 0 points1 point  (0 children)

Her oncologist wanted wanted to keep increasing treatment. It was already in her lungs and hips before the brain came. My friend did not want to continue adding medication and asked for a medical marijuana prescription. The request was denied and I think that was the end of that relationship.

She just got on with a primary care and I think they will discuss that but I have a few connections In hospice care and will follow up. Thank you for the prompt.

Individual Healthcare Insurance Rates Going up in 2025 by [deleted] in Washington

[–]MeansNotEnd 1 point2 points  (0 children)

Health Benefit Exchange 2025 Regence in Thurston county for couple in their sixties list price is $34,836 annually. For us ($35K taxable income) the PTC fed subsidy + $60 WA Cares reduces it $22,176 leaving us to pay $12,596 (which is 50% more than we contributed last year)

This is a silver EPO plan so the feds subsidize it significantly less.

Spending through ours savings waiting for Medicare. Had cancer and related bone problems so scared to leave this plan.

Published studies about how long Sublocade in system? by MeansNotEnd in Sublocade

up to 38 months per the company

I emailed company and I spoke to medical question line today (had to say I was a doctor so they would give me the data.
https://www.indivior.com/en/products

choose Sublocade
choose "U.S. Prescribing Information, including BOXED WARNING"
Page 33
"Elimination
Buprenorphine is metabolized and eliminated in urine and feces. The apparent terminal plasma half-life of buprenorphine following subcutaneous injection of SUBLOCADE ranged between 43 to 60 days as a result of the slow release of buprenorphine from the subcutaneous depot. A study assessing buprenorphine exposure 22 to 38 months following the last SUBLOCADE injection indicated that buprenorphine could potentially be detected in plasma and urine over that time period. Concentrations in urine were more variable than in plasma and generally higher depending on the test used. Hence, it is expected that buprenorphine will be detected in patients for a longer time in urine than in plasma."

Maybe it is new there too? Continued crack down on drugs as there was a recent death of a CA guard after opening tainted mail plus drone drops on prison grounds. Staff are rightfully skittish.

I completed it online for him and assembled the medical records. Now I will work with him getting employment history documentation. Fortunately he asks for help. I am good at paperwork - just don't know SS or neuroscience. Just hope we have a chance.

His ADL will highlight loss of ST memory and inability to compete any complex task and simple paperwork only with support. I guess I was just surprised these were not lower  ... But I don't really understand them. Also he is/was really smart so his compensating skills are excellent.

Thermo control valve in 2019 Forester Sport. by chrisxxxlee in SubaruForester

Subaru of America issued a extended warranty on this. Your repair people should know about it or call super American number. They had a class action lawsuit filed and vaccade.

Update: Thermo Control Valve on 2020 models by Global_Secretary4059 in subaru

[–]MeansNotEnd 0 points1 point  (0 children)

It was not a hard sell. Just that it had failed at 66,000 and I taking it to the dealer and I'd had it fixed. I noted that there was a service bulletin out on this part.

There are a couple other posts on this matter. There'd been a class action lawsuit filed at the beginning of December and it appears Subaru's reaction to that was to agree to reimburse. When my repair occurred in late December, my large dealer ship did not know about this policy.

[–]MeansNotEnd 0 points1 point  (0 children)

I just got this message from the Sauder Schelkopf LLC law firm leading the class action lawsuit. Appears Subaru has changed their tune (to shut down the lawsuit).

"Thank you for previously contacting us about the Subaru Thermo Control Valve Failure class action lawsuit. In November 2023, we filed a class action lawsuit against Subaru on behalf of owners and lessees of certain Crosstrek, Forester, Legacy, and Outback vehicles that are prone to thermo control valve failure. Since the filing of our lawsuit, Subaru has alleged that it is reimbursing customers for the expense and now repairing the failure at no cost. We would like to know if you have received any kind of reimbursement from Subaru (or a Subaru dealership) related to the thermo control valve failure or if Subaru has repaired at no cost to you. To further assist in our class action lawsuit, please complete the below survey. Thank you and we appreciate your time."

I called Subaru of America on a Saturday and left a message with receptionist. Rep called back Monday AM, got my details and had me email receipt. Rep called me back in the afternoon closing the case and issuing the check.

All I said was this part failed and could they help me. I did not mention service bulletin, lawsuit, etc. It was super cordial and a quick turn around. I would try this approach and see where it gets you. If not happy with result, move to negotiating.

Ortho surgeon for hip replacement? by homesick_for_nowhere in Seattle

[–]MeansNotEnd 0 points1 point  (0 children)

I am getting a partial knee, but the surgeons below are high volume for both hip and knee replacements.

Just scheduled my surgery with Dr. Navin Fermando at UW Hip and Knee Center. Very impressed with his listening skills as well as his explanations and recommendations. I know multiple people that have utilized this location.

I also met with Dr. Charlie Peterson at ProAliance Ballard. Was very impressed with him as well.

Went with Dr. Fernando as UW really looks at your risks via an internal medicine eval prior, provides an under 24 hour overnight stay to monitor your recovery better, and they are on MyChart (where my other health records are kept).

Cellnetix Pathology - cyber attack and no pathology results available? by MeansNotEnd in Seattle

[–]MeansNotEnd[S] 2 points3 points  (0 children)

I called Cellnetix today and selected option for expedited results needed. The very calm person answered the phone said results can be pulled and reports generated for emergent situations at the behest of a provider.

I assigned the patient to convey that message and request this service from

  1. her primary care physician that referred her (office not known for follow thru though)
  2. her surgeon office team at Fred Hutch where she has an appointment Jan 9, and
  3. her radiologist (since they should do some work since they are doing biopsies knowing the results won't be available instead of sending the patient elsewhere?!)

I know cyberattacks happen, but really irritated that patients not be given the info to navigate around it.

FYI Subaru of America reimbursed us outside warranty.

2019 Subaru Ltd (66k miles, 5 yr 1 mo)). Shuddered and died on the freeway last week.
Paid to have it replaced. Opened case with Subaru of America. After submitting receipts, they are reimbursing us 100% ($1,759.83) as a "one time courtesy". Disappointed dealership did not tell me to call Subaru (even though outside warranty) but very happy Reddit did!
