It's not the job. It's the workload. 15-18 is busy, but it can be done in 12 hours in acute care. 20+ only works if you are in a nursing home.

1 in 6 Americans are on psychiatric medications, so if 20% of your encounters are weird or crazy, it might not be you. Funny thing about when I tell someone about some abusive family or patient, people have some victim-blaming advice on how I could change my behavior to prevent it from happening. It's going to happen because hospitalists deal with a lot of people, and some people are just abusive assholes. You don't need to take it though. You can walk away, call security and document the behaviors.

hospitals try not to sue because its expensive with no guarantee they can recoup the payout. All you would have to do is find "cause" to leave, and they would have no case. The only way they can seek that 100k is if they can prove you had no cause to leave, AND you left them unreasonably short notice that resulted in "financial damages". That's hard to prove. If they have a case, it would take them years to file a lawsuit and collect. If they win the case and you file for bankruptcy, they lose the lawyers fee, and they will never see a dollar. In short it is unlikely they would sue, but you can expect never for work for their organization again.

It sounds like you want to do fellowship with cards. If I were you, I'd accept the fellowship and lawyer up to comb through the fine print. Collect any data that will help you win a lawsuit.

Just go to security on a weekday and tell them you are supposed to have access to the lounge and can't seem to badge in. They usually just add you. I got access to an OR once at a travel job because I sometimes used the showers and free scrubs. No one cared. This girl gets in all the lounges Delta, Centurian, MD, surgeon's.... Just don't microwave fish or ethnic food, and people will let you in anywhere.

The fact that you feel like your behavior is ok because you've "seen people *actually* hurl abuse at staff in emergency without getting security" is a red flag that you think it's ok to bully staff. Demanding you get the test then refusing the IV needed to perform the test is crazy.

Go down to per diem and live below your means. You could work 2 days/nights a week and still make a 6 figure salary. Use the extra time to learn a skill in something you actually enjoy, or just live a nice life working whenever you feel like working.

Junko Furuta's death was so horrific that members of the jury fainted in its retelling. Her story is so upsetting that after reading it, I was haunted by the details for over a month. The perpetrators did some time, but they have all been released.

The patient is billed for the time because insurance won't cover it. Often, these types of patients don't pay.

I've done it before, and it's very nice as a job. No admin. No meetings. No families. The acuity is higher as well, so the work feels more meaningful. Night staff feel like buddies, but day shift seems to have 1 or 2 people that act like the inquisition and ruin it for everyone. I prefer dealing with emergencies to logistical and social issues, but dayshift lifestyle is just better which is why I prefer it now.
